Product Overview

Introduction of Xepa Aslene 120mg Capsule 30s

Xepa Aslene 120mg Capsule is a locally manufactured prescription medicine belonging to the gastrointestinal lipase inhibitor drug class, with orlistat as its active pharmaceutical ingredient — produced by Xepa-Soul Pattinson (Malaysia) Sdn Bhd in Melaka and registered with the National Pharmaceutical Regulatory Authority (NPRA) of Malaysia. As with all medicines, Xepa Aslene 120mg can cause side effects, and the most commonly reported ones are gastrointestinal in nature — including oily or fatty stools, oily spotting on underwear, increased urgency to pass stools (sometimes with an inability to control bowel movements), and flatulence with oily discharge — effects that are generally more pronounced when high-fat meals are consumed. It is important that you take this medicine only under the supervision of a registered doctor, follow a low-fat, calorie-controlled diet as directed, inform your healthcare provider of all other medications you are currently taking, and take a daily fat-soluble multivitamin supplement (containing Vitamins A, D, E, K, and beta-carotene) at a different time of day from Aslene — as this medicine reduces the absorption of fat-soluble vitamins. As a prescription-only medicine in Malaysia, you will need a valid prescription from an MMC-registered doctor before purchasing, and you should only buy from a licensed pharmacy or verified online platform to ensure you receive a genuine, NPRA-registered product.

How to Take Xepa Aslene 120mg Capsule

Xepa Aslene 120mg Capsule is taken orally — swallow the capsule whole with a full glass of water during a meal or up to one hour after a meal that contains fat; do not chew or crush the capsule. If a meal is missed, or if the meal you are eating contains no fat at all, you should skip that particular dose of Aslene — there is no need to take the capsule if there is no dietary fat present for it to act upon. Always follow your doctor’s specific instructions regarding dosing timing, dietary guidelines, and the use of a daily fat-soluble vitamin supplement to be taken separately from this medication.

Dosage of Xepa Aslene 120mg Capsule

The recommended dose for adults and adolescents aged 12 years and above is one capsule (120mg) taken three times daily — with each of the three main fat-containing meals of the day (breakfast, lunch, and dinner) — with a maximum of three capsules (360mg) in any 24-hour period; doses higher than 120mg three times daily have not been shown to provide additional clinical benefit. This medication is generally not recommended for children under 12 years of age, and patients with organ transplants, renal impairment, or hepatic impairment should only use Aslene under strict medical supervision with dose and safety adjustments as determined by their doctor.

Shelf Life of Xepa Aslene 120mg Capsule

Xepa Aslene 120mg Capsule has a manufacturer-stated shelf life clearly printed on both the outer carton and individual blister strip packaging — always check this date before taking a capsule. Do not consume this medication after the stated expiration date, even if the capsule appears visually unchanged, as chemical and physical degradation over time may affect the potency and safety profile of the product. Properly stored, unopened blister packs will retain their full pharmaceutical quality for the duration printed on the manufacturer’s packaging. Once dispensed by a pharmacy, always observe any expiry or “use by” date applied to the dispensing label, which may in some cases be earlier than the manufacturer’s stated shelf life.

How to Store Xepa Aslene 120mg Capsule

Store Xepa Aslene 120mg Capsule below 25°C in a cool, dry place, away from direct sunlight, heat, and moisture. Keep the capsules in their original blister packaging until you are ready to take each dose — the sealed foil-blister packing protects the capsule shells and their contents from humidity and light exposure, both of which can compromise the stability of hard gelatin capsules.

In Malaysia’s warm, humid tropical climate, take special care not to store this medication in the bathroom, near bathroom sinks, on kitchen surfaces adjacent to stoves or kettles, on bedroom windowsills that receive afternoon sun, or inside vehicles where temperatures can reach dangerous levels during the day. Even brief repeated exposure to heat and humidity can cause gelatin capsule shells to soften, deform, or stick together, potentially affecting dosage accuracy.

Keep all medicines, including Xepa Aslene 120mg, securely out of sight and reach of children and pets at all times. Dispose of any unused, damaged, or expired capsules through a licensed pharmacy medication take-back programme or in accordance with local pharmaceutical waste disposal guidelines — do not discard in household rubbish bins or flush down the drain.

Active Ingredient of Xepa Aslene 120mg Capsule

The active pharmaceutical ingredient (API) in each Xepa Aslene 120mg Capsule is Orlistat (also known by its chemical name tetrahydrolipstatin), present at a dose of 120mg per capsule. Orlistat is a synthetic, saturated derivative of lipstatin — a naturally occurring compound originally isolated from Streptomyces toxytricini — and is classified as a peripherally acting anti-obesity agent under the ATC code A08AB01. It is one of the most extensively studied weight management agents in pharmaceutical medicine.

Pharmacodynamically, orlistat works by reversibly inhibiting gastric and pancreatic lipases — the digestive enzymes responsible for breaking down dietary fat (triglycerides) into smaller, absorbable components (free fatty acids and monoglycerides) within the lumen of the stomach and small intestine. Orlistat achieves this by forming a stable covalent bond with the serine residue at the active site of these lipase enzymes, thereby rendering them temporarily inactive. When the enzymes cannot function, the triglycerides in your food remain in their intact, undigested form and pass through the gastrointestinal tract without being absorbed, exiting the body in the stool. At the recommended therapeutic dose of 120mg three times daily, orlistat inhibits approximately 30% of dietary fat absorption. Crucially, because orlistat acts locally within the gut and is not significantly absorbed into the systemic circulation, it does not exert CNS effects or interact with systemic metabolic pathways in the way that centrally acting anti-obesity medicines do.

Pharmacokinetically, systemic absorption of orlistat is minimal — plasma concentrations following oral administration are extremely low (nanomolar range) and essentially negligible. The majority of the drug acts entirely locally within the gastrointestinal lumen. Any absorbed fraction undergoes rapid metabolism within the gut wall and liver. Faecal excretion of unabsorbed orlistat is the primary elimination pathway, with approximately 97% of the dose recovered in the stool (as drug and metabolites), and less than 2% excreted in urine. Due to its minimal systemic absorption, orlistat has a very limited systemic pharmacokinetic profile compared to most other prescription medicines, which is one of the reasons its drug interaction profile is primarily centred on gastrointestinal absorption interference rather than systemic metabolic pathways.

In addition to orlistat 120mg, each capsule of Xepa Aslene contains the following inactive excipients: microcrystalline cellulose, sodium starch glycolate, povidone, sodium lauryl sulphate, and talc within the capsule fill; the hard gelatin capsule shell contains gelatine, titanium dioxide (E171), FD&C Blue No. 1 (Brilliant Blue FCF, E133), FD&C Blue No. 2 (Indigo Carmine, E132), and other colourants that give the capsule its distinctive blue colour. Patients with known allergies to gelatine or any of the excipients listed should consult their doctor or pharmacist before use.

Side Effects of Xepa Aslene 120mg Capsule

As with all medicines, Xepa Aslene 120mg Capsule may cause side effects in some patients. The majority of side effects associated with orlistat are gastrointestinal in nature and are a direct pharmacological consequence of unabsorbed fat passing through the digestive tract. These gastrointestinal effects are generally more frequent and pronounced when dietary fat intake is higher than recommended — keeping fat intake below 30% of total caloric intake per meal significantly reduces their occurrence. The following side effects have been reported with orlistat:

Very Common Gastrointestinal Side Effects (may affect more than 1 in 10 people):

  • Oily spotting or staining on underwear — oily discharge from the rectum between bowel movements
  • Flatus with oily discharge — passing wind accompanied by an oily or fatty leakage
  • Faecal urgency — sudden, urgent need to have a bowel movement with limited time to reach a toilet
  • Fatty or oily stools (steatorrhoea) — stools that appear greasy, float, or leave an oily residue in the toilet
  • Oily evacuation — oily material passed during a bowel movement
  • Increased frequency of bowel movements (increased defaecation)
  • Faecal incontinence — involuntary or uncontrolled passing of stools; this may occur particularly during the initial weeks of treatment or after a high-fat meal
  • Soft stools
  • Liquid stools or diarrhoea
  • Abdominal pain, discomfort, or cramping
  • Nausea

Common Side Effects (may affect up to 1 in 10 people):

  • Headache
  • Upper respiratory tract infection (e.g., colds or mild respiratory illness) — noted in clinical trials
  • Influenza (flu-like symptoms)
  • Fat-soluble vitamin deficiency — orlistat reduces the absorption of fat-soluble vitamins (A, D, E, K) and beta-carotene; prolonged use without supplementation can lead to clinically significant deficiencies of these vitamins, affecting vision (Vitamin A), bone health (Vitamin D), immune function (Vitamin E), and blood clotting (Vitamin K)
  • Hypoglycaemia — blood glucose may decrease in patients with diabetes mellitus as weight loss improves insulin sensitivity; antidiabetic medication dosage may require adjustment

Uncommon to Rare Side Effects:

  • Hepatitis — rare cases of serious liver injury (hepatotoxicity) have been reported in patients taking orlistat; although a definitive causal relationship has not been established in all reported cases, patients should be alert to signs of liver dysfunction
  • Cholelithiasis (gallstones) — weight loss in general is associated with an increased risk of gallstone formation; orlistat-assisted weight loss may carry this risk
  • Nephrolithiasis (kidney stones) and hyperoxaluria — orlistat reduces fat absorption, which can increase oxalate absorption from the gut; elevated urinary oxalate levels may promote the formation of calcium oxalate kidney stones, particularly in patients with a history of renal disease or prior kidney stones
  • Pancreatitis — rare reports have been received through post-marketing surveillance; a definitive causal relationship between orlistat and pancreatitis has not been established, but patients should seek medical attention for persistent severe upper abdominal pain
  • Hypothyroidism — reported in patients taking orlistat concomitantly with levothyroxine, due to impaired levothyroxine absorption
  • Rectal pain or discomfort
  • Tooth and gum disorders — observed in clinical trials; exact mechanism not clearly established
  • Menstrual irregularities — rare; possibly related to significant changes in body weight and hormone levels
  • Anxiety — rare; noted in post-marketing reports
  • Seizures — reported in patients taking orlistat with antiepileptic drugs

Serious Side Effects — Stop Medication and Seek Immediate Medical Attention:

  • Signs of serious liver injury (hepatotoxicity) — yellowing of the skin or whites of the eyes (jaundice), dark urine, pale stools, severe upper right abdominal pain, persistent nausea, vomiting, or loss of appetite; stop taking Aslene immediately and seek urgent medical review
  • Severe allergic reaction (anaphylaxis or angioedema) — sudden difficulty breathing, facial swelling, swelling of the lips, tongue or throat, widespread urticaria (hives), or collapse; seek emergency medical care immediately
  • Signs of pancreatitis — severe, persistent abdominal pain that may radiate to the back, accompanied by nausea and vomiting; seek emergency medical attention

Contraindications of Xepa Aslene 120mg Capsule

Xepa Aslene 120mg Capsule must NOT be used in the following patients and circumstances:

Absolute Contraindications — Do Not Use:

  • Known hypersensitivity or allergy to orlistat or to any inactive excipient in the Xepa Aslene 120mg Capsule formulation — including gelatine, sodium lauryl sulphate, or any of the colourants (FD&C Blue No. 1 and No. 2)
  • Chronic malabsorption syndrome
  • Cholestasis
  • Pregnancy
  • Breastfeeding
  • Children under 12 years of age
  • Organ transplant recipients taking immunosuppressants (e.g., cyclosporine, tacrolimus)

Use with Caution — Requires Medical Review and Close Monitoring:

  • Type 1 or Type 2 diabetes mellitus
  • Hypothyroidism or patients taking levothyroxine
  • Renal (kidney) impairment
  • Hepatic (liver) impairment
  • Patients with a history of gallstones or cholelithiasis
  • Patients with hyperoxaluria or a history of calcium oxalate nephrolithiasis
  • Patients with eating disorders
  • Patients taking anticoagulants such as warfarin
  • Patients taking antiepileptic medications
  • Patients taking antiretroviral HIV medications

Drug & Food Interaction of Xepa Aslene 120mg Capsule

Because orlistat acts primarily within the gastrointestinal lumen rather than the systemic circulation, its drug interactions are largely mediated through impaired gastrointestinal absorption of co-administered medications — rather than through the hepatic CYP450 enzyme interactions more commonly seen with systemically absorbed drugs. This makes separating dose timings a key management strategy for several important interactions.

Critical Drug Interaction — Cyclosporine (Immunosuppressant for Organ Transplant Patients):

Drug Interactions Requiring Dose Separation and Monitoring:

  • Levothyroxine — take at least 4 hours apart from Aslene
  • Warfarin and other anticoagulants — monitor INR closely
  • Antiepileptic drugs — monitor seizure frequency and drug levels
  • Amiodarone — monitor antiarrhythmic effect
  • Antiretroviral HIV medications — monitor viral load, consult HIV specialist
  • Fat-soluble vitamin supplements — take 2 hours before or after Aslene, or at bedtime
  • Antidiabetic medications — monitor blood glucose and adjust doses if necessary
  • Other anti-obesity agents — avoid unless directed by a doctor

Dietary Interactions and Food Guidance:

  • Dietary fat content per meal — keep below 30% of total calories per meal
  • Alcohol — limit intake
  • Grapefruit juice — generally safe with orlistat, follow guidance for other medications
  • Very low-fat or fat-free meals — skip Aslene dose if meal contains no fat

For patients on ongoing therapy, we recommend planning ahead and contacting our pharmacist team to discuss your supply needs. Please note that you may be required to undergo periodic medical reviews as part of responsible long-term weight management, and your doctor may request a repeat consultation before issuing further prescriptions.

Q10: What should I do if my Aslene order arrives damaged, tampered, or incorrect?

Do not consume any medication that has arrived with damaged, unsealed, or tampered packaging, or if the wrong product has been delivered. Contact our customer service and pharmacist team immediately, providing photographs of the product and packaging together with your order reference number. We will investigate and arrange an appropriate resolution — including a replacement shipment, exchange, or refund as applicable — in line with our pharmaceutical product handling and patient safety policy.

Q11: Can I return Xepa Aslene 120mg Capsule after it has been dispensed?

In accordance with Malaysian pharmacy practice standards and regulatory patient safety requirements, dispensed prescription medicines are generally not accepted for return or exchange once they have left the pharmacy — due to patient safety, product integrity, and storage compliance considerations. The only exceptions are products that arrive damaged, past expiry, or incorrectly dispensed. If your order qualifies for a return or replacement under these conditions, please contact our team promptly and we will guide you through the resolution process.

Q12: How do I identify a legitimate, safe online pharmacy to order from in Malaysia?

A genuine, regulated online pharmacy in Malaysia should: require a valid prescription for all prescription-only medicines; employ licensed, registered pharmacists who review every order; sell only NPRA-registered pharmaceutical products; comply fully with Malaysian pharmacy legislation, healthcare regulations, and the Medicines Advertisement Board (MAB) requirements; and provide clear contact information, registered business details, and accessible pharmacist credentials. Our platform meets all of these standards. You can verify pharmacy licences with the Pharmacy Board Malaysia (Lembaga Farmasi Malaysia — LFM) at www.pharmacy.gov.my. Avoid any platform selling prescription medicines without requiring a valid prescription, offering prices that seem unrealistically low, or operating without clearly verifiable regulatory credentials.

Q13: Are there other orlistat 120mg products available in Malaysia besides Xepa Aslene?

Yes. The originator brand Xenical 120mg Capsule (Roche / Chugai Pharma) is also available in Malaysia and contains the same active ingredient (orlistat 120mg) at the same strength. Several other NPRA-registered orlistat 120mg products may also be available in the Malaysian pharmaceutical market. Our pharmacist team can advise you on current availability, pricing differences, and registered equivalents to help you make an informed choice in consultation with your prescribing doctor.
