Concor 5mg Tablet 100s
RM 213.00
Concor 5mg Tablet is a prescription-only medicine that belongs to the cardioselective beta-1 adrenoceptor blocker drug class — more commonly known as a beta-blocker — and contains Bisoprolol Fumarate as its active ingredient, a highly selective beta-1 blocking agent manufactured by Merck KGaA and registered with Malaysia's National Pharmaceutical Regulatory Agency (NPRA). Bisoprolol is listed on the World Health Organization's List of Essential Medicines, reflecting its long-established and globally recognised clinical importance in cardiovascular medicine. Like all beta-blocker medicines, Concor 5mg can cause side effects, and the ones most commonly reported include fatigue or tiredness, dizziness, headache, and cold hands or feet — all of which are generally mild and linked to the medicine's expected effect of slowing the heart rate and reducing blood pressure. Before you start taking this tablet, there are a few important things you should be aware of: Concor 5mg must never be stopped abruptly without your doctor's guidance — doing so can trigger a rebound effect that may cause a dangerous surge in heart rate and blood pressure; you should also let your doctor know if you have asthma, significant breathing problems, or diabetes, as Bisoprolol can mask the warning signs of low blood sugar and, despite its cardioselectivity, may still affect the airways at higher doses.
Concor 5mg Tablet is taken orally — swallow the tablet whole or halved along the score line if half a tablet is prescribed, with a glass of water, and it may be taken with or without food as food does not significantly affect Bisoprolol absorption. It is best taken at the same time each day, typically in the morning, to maintain consistent plasma levels and to help you build a reliable daily routine with your medication. Never crush or chew the tablet, and do not miss doses — if you do miss a dose and it is still the same day, take it as soon as you remember; if it is almost time for your next scheduled dose, skip the missed dose and resume your normal schedule without doubling up.
The usual starting dose for most adult patients is 5mg of Bisoprolol Fumarate once daily, which may be adjusted by the prescribing doctor up to a maximum of 20mg once daily depending on the condition being treated, individual response, and tolerability — for the management of stable chronic heart failure, a specific low-dose titration protocol starting at 1.25mg daily is used, with very gradual upward adjustments over several weeks under careful specialist supervision. Always take exactly the dose prescribed by your doctor and never adjust it independently, as both under-dosing and over-dosing with beta-blockers carry clinically significant cardiovascular risks.
Always check the expiry date printed on both the outer carton and the individual blister strip before taking each tablet. The chemical and pharmacological integrity of Bisoprolol Fumarate cannot be guaranteed beyond the manufacturer's stated shelf life — do not use any tablet that has passed its expiry date. Expired tablets should be returned to a licensed pharmacy for safe pharmaceutical disposal; do not discard them in household waste or flush into drainage systems.
Bisoprolol Fumarate is a second-generation, highly selective beta-1 adrenoceptor antagonist that blocks the binding of endogenous catecholamines — primarily adrenaline (epinephrine) and noradrenaline (norepinephrine) — to beta-1 adrenergic receptors located predominantly in cardiac tissue. This selectivity for beta-1 over beta-2 receptors is one of Bisoprolol's defining pharmacological characteristics: it means that at standard therapeutic doses, Bisoprolol has a relatively low tendency to affect beta-2 receptors in the bronchial and vascular smooth muscle compared to non-selective beta-blockers, making it the preferred beta-blocker choice in patients with mild-to-moderate airway disease — though it is still not considered safe in severe bronchial asthma. Bisoprolol lacks intrinsic sympathomimetic activity (ISA) and does not have clinically relevant membrane-stabilising properties. Its pharmacokinetic profile is balanced: approximately 50% of the absorbed dose undergoes hepatic first-pass metabolism, with the remaining active drug excreted renally — meaning both renal and hepatic function influence its clearance and may require dose adjustment in severe organ impairment. The inactive excipients in Concor 5mg Tablet include: anhydrous colloidal silica, magnesium stearate, crospovidone, microcrystalline cellulose, maize starch, and anhydrous calcium hydrogen phosphate (tablet core); and iron oxide yellow, dimethicone, macrogol 400, titanium dioxide, and hypromellose (film coating) — refer to the full product package insert for the complete formulation declaration.
Side effects may occur with Concor 5mg Tablet. The following have been documented in clinical trials and post-marketing surveillance for Bisoprolol Fumarate:
Concor 5mg Tablet must not be used in the following circumstances:
Use with caution in patients with first-degree AV block, Prinzmetal's angina, peripheral arterial disease, poorly controlled diabetes, psoriasis, restrictive cardiomyopathy, recent myocardial infarction, and during general anaesthesia.
