Glomerular Filtration Rate (GFR) problem

Dear Doctor,
Good morning, I am 32 years old male.
On September 2020, I got my result of blood and urine test. Every result is good and I have no any problems like high blood pressure, obesity or diabetes, however my gfr is about 79% only and the report shown mild decrease of kidney function.

After that i take precaution and extra steps to take care on my own health including regular exercise, keep hydrated, less salty food intake and take supplement such as Bio-Lingzhi 4 tablets daily + Waist Tonic 60ml per perday for about 3 months

On 15 Jan 2021, i got my next result for blood and urine test and it shows further reduced of 1% for gfr to 78%. I felt deep sadness and disappointed for the result after so much of effort being taken. Afterward, my panel doctor ask me to take “ketosteril tablets” 1 pill per day.

My question is:

  1. Is that 78% of gfr result is ok? some info from the web mention 60-120 is normal however some info mentioned above 90 only considered normal.
  2. Is that anyway to improve gfr or it will never back to normal after damage for example in my case is that possible to increase gfr percentage above 90%?
  3. Is that suggestable to take CT scan to check on the real cause of reduction for GFR?

Dear Dr, your kind assistance will be highly appreciated.
Thank you very much.

Greetings! Thank you for your questions.

First and foremost, we truly understand your concerns regarding your symptoms and other health problems you might be facing. For your information, the estimated glomerular filtration rate is the best test to measure your level of kidney function and determine your stage of kidney disease. To diagnose a chronic kidney disease, it requires a GFR below 60 for three months or more or GFR above 60 with kidney damage (marked by high levels of albumin in your urine) indicates chronic kidney disease. Your doctor will want to investigate the cause of your kidney disease and continue to check your kidney function to help plan your treatment. Chronic kidney disease (CKD) refers to all five stages of kidney damage, from very mild damage in stage 1 to complete kidney failure in stage 5. Patients who are diagnosed with CKD with GFR of 78% means that they have CKD in stage 2 which shows kidney damage with mild loss of kidney function.

According to the National Kidney Foundation, normal results range from 90 to 120 mL/min/1.73 m2. Older people will have lower than normal GFR levels, because GFR decreases with age.

Answering your second question, if the decrease in the estimated glomerular filtration rate is due to acute kidney injury with a sudden decrease in kidney function, this can commonly be reversed. If the kidney disease is due to chronic kidney disease (CKD), the recovery of GFR is usually not possible.

Furthermore, CT scans of the kidneys are useful in the examination of one or both of the kidneys to detect conditions such as tumors or other lesions, obstructive conditions, and the location of abscesses. However, this investigation will only be suggestable based on the doctor’s decision after physical examination.
