Overview

Januvia 50mg Tablet contains sitagliptin 50mg, a dipeptidyl peptidase-4 (DPP-4) inhibitor. It is used, together with diet and exercise, to improve blood glucose control in adults with type 2 diabetes mellitus, either alone or in combination with other glucose-lowering medicines such as metformin, sulfonylureas, or insulin.

Sitagliptin works by blocking the DPP-4 enzyme, which normally breaks down incretin hormones (such as GLP-1) in the body. By preserving these hormones for longer, sitagliptin increases insulin release from the pancreas and reduces glucagon release in a glucose-dependent manner, helping to lower blood glucose mainly after meals.

How it Works

Sitagliptin inhibits the DPP-4 enzyme, which is responsible for the rapid breakdown of incretin hormones such as GLP-1 and GIP that are released after eating. By slowing this breakdown, more active incretin hormone remains in circulation, enhancing glucose-dependent insulin secretion from the pancreas and suppressing inappropriate glucagon release, which together lower post-meal blood glucose without directly stimulating insulin release when glucose levels are already normal.

Directions for Use

Take Januvia 50mg Tablet once daily, with or without food, swallowing the tablet whole with a glass of water. Continue taking this medicine regularly even when you feel well, and do not stop it without consulting your doctor, as diabetes control needs to be maintained continuously.

Dosage

The usual dose of Januvia 50mg Tablet is 100mg once daily, taken alone or together with other diabetes medicines. Your doctor may prescribe a lower dose if you have reduced kidney function, and will individualise your overall diabetes treatment plan based on your blood glucose control and other health conditions.

Shelf Life

Use this product before the expiry date stated on the packaging. Do not use the medicine if the expiry date has passed.

Side Effects

  • Headache
  • Nasal congestion or sore throat
  • Stomach discomfort or indigestion
  • Joint pain
  • Mild skin rash
  • Low blood sugar (especially if taken with other diabetes medications)

Contraindications

  • Known hypersensitivity to sitagliptin or any ingredient in this product, or a history of a serious hypersensitivity reaction to any DPP-4 inhibitor (e.g. anaphylaxis, angioedema, or severe skin reactions)
  • Type 1 diabetes mellitus (not indicated for this use)
  • Diabetic ketoacidosis

Drug / Food Interactions

  • Insulin or sulfonylureas may increase the risk of low blood sugar (hypoglycaemia) when combined – a lower dose of these medicines may be needed
  • Digoxin levels may be slightly increased and monitoring may be advised if combined
  • Inform your doctor of all other medicines you are taking, particularly other diabetes medicines

Quick Tips

  • Take at the same time each day, with or without food
  • Monitor your blood glucose regularly as advised by your doctor
  • Seek urgent medical attention for severe or persistent abdominal pain, which can rarely signal pancreatitis
  • Report any new joint pain to your doctor, as this has rarely been reported with DPP-4 inhibitors
  • Continue taking this medicine as prescribed even if you feel well, since diabetes often has no symptoms

Frequently Asked Questions

What is Januvia 50mg Tablet used for?

Januvia 50mg Tablet is used to improve blood glucose control in adults with type 2 diabetes, either alone or together with other diabetes medicines, alongside diet and exercise.

Does this medicine cause weight gain?

Sitagliptin is generally considered weight-neutral and, unlike some other diabetes medicines, is not typically associated with significant weight gain. Your overall weight will still depend on diet, activity level, and any other medicines you are taking.

Can this medicine cause low blood sugar?

When used alone, sitagliptin has a low risk of causing hypoglycaemia because its effect on insulin release depends on blood glucose levels being elevated. The risk increases when it is combined with insulin or a sulfonylurea, so your doctor may lower those doses.

What should I watch out for regarding my pancreas?

Rarely, DPP-4 inhibitors like sitagliptin have been associated with pancreatitis. Seek urgent medical attention if you develop severe, persistent abdominal pain, especially if it spreads to your back and is accompanied by nausea or vomiting.

Can I take this medicine if I have kidney problems?

Yes, but your doctor will likely prescribe a lower dose if you have moderate to severe kidney impairment, since sitagliptin is cleared mainly through the kidneys. Always inform your doctor of your kidney function and any changes to it.

What should I do if I miss a dose of Januvia 50mg Tablet?

Take the missed dose as soon as you remember, unless it is almost time for your next dose – in that case, skip the missed dose and continue as normal. Do not double the dose.
