Overview

Apo-Warfarin 2mg Tablet is a prescription-only medicine (POM) that contains Warfarin 2mg (as warfarin sodium). It is supplied as a tablet and is dispensed in Malaysia only against a valid prescription issued by a registered doctor.

Under Malaysian rules for prescription medicines, this page carries product, safety and handling information only. Your doctor or pharmacist will explain whether this medicine is suitable for you, the dose you should take, and how long you should take it.

On DoctorOnCall, Apo-Warfarin 2mg Tablet is reviewed by a licensed doctor through an online consultation and dispensed by a registered pharmacist before delivery. Always follow the instructions on your prescription label and the package insert supplied with the product, and do not share this medicine with anyone else.

Directions for Use

Take Apo-Warfarin 2mg Tablet at the same time each day, usually in the evening, with or without food. Try to keep your day-to-day intake of vitamin K-rich foods (such as leafy green vegetables) reasonably consistent rather than avoiding them altogether, as sudden changes can affect how the medicine works. Never skip or double a dose, and attend every scheduled INR blood test.

Dosage

The dose of Apo-Warfarin 2mg Tablet is highly individualised and based on regular blood tests (INR – International Normalised Ratio) that measure how quickly your blood clots. Never adjust your dose yourself – your doctor will determine and adjust your dose to keep your INR within your specific target range.

Storage Instructions

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

Side Effects

  • Bleeding or bruising easily
  • Nosebleeds or bleeding gums
  • Nausea or loss of appetite
  • Hair thinning
  • Purple toes syndrome (rare)
  • Fatigue
  • Abdominal cramps

Contraindications

  • Known hypersensitivity to warfarin
  • Active, clinically significant bleeding
  • Pregnancy (warfarin can cause serious birth defects, especially in the first trimester; use in pregnancy is only ever considered by a specialist in specific situations such as a mechanical heart valve)
  • Recent major surgery or a high bleeding-risk procedure
  • Severe uncontrolled hypertension or conditions with a high bleeding risk, such as active peptic ulcer disease

Drug / Food Interactions

  • Many antibiotics (e.g. metronidazole, fluoroquinolones, macrolides) and antifungals (e.g. fluconazole) can significantly increase your INR and bleeding risk
  • NSAIDs and aspirin increase the risk of bleeding when combined with warfarin
  • Sudden large changes in intake of vitamin K-rich foods (leafy greens such as kale, spinach, broccoli) can alter warfarin’s effect – keep your intake consistent rather than avoiding these foods
  • Many herbal supplements (e.g. St John’s Wort, ginkgo) and alcohol can also affect your INR unpredictably – always check with your doctor or pharmacist before starting anything new

Quick Tips

  • Attend every scheduled INR blood test without fail
  • Never change your dose on your own – always follow your doctor’s instructions after each INR result
  • Keep your vitamin K intake (leafy greens) roughly consistent rather than cutting it out suddenly
  • Tell every healthcare provider, including your dentist, that you are taking warfarin before any procedure, and carry an anticoagulant alert card
