Ozempic Dosage Guide: 0.25mg, 0.5mg, 1mg & 2mg Pens Explained

Introduction
Ozempic Dosage Guide: 0.25mg, 0.5mg, 1mg & 2mg Pens Explained
Ozempic is taken once a week, starting at 0.25mg and stepping up in 4-week stages to a maintenance dose of 0.5mg, 1mg or 2mg. Your doctor decides which dose you stay on based on your blood sugar control and how well you tolerate it.
This guide explains each dose, the schedule for moving between them, and which Ozempic pen delivers which dose, so you know exactly what your prescription means before you order.
What is Ozempic?
Ozempic contains semaglutide, a GLP-1 receptor agonist. It is injected under the skin once a week and is registered in Malaysia for adults with type 2 diabetes, alongside diet and exercise. It helps the pancreas release insulin when blood sugar is high and slows how quickly the stomach empties.
Ozempic is a prescription-only medicine. In Malaysia you need a valid prescription from a registered doctor before a pharmacy can dispense it. A DoctorOnCall doctor can assess you online and, if suitable, issue the prescription.
Ozempic dose schedule
Ozempic dose schedule: how the dose steps up
Everyone starts on 0.25mg. The dose rises only if your doctor decides you need more blood sugar control, and never faster than once every 4 weeks.
| Stage | Weekly dose | How long | Purpose | Pen used |
| 1. Starting dose | 0.25mg | Weeks 1–4 | Lets your body adjust; reduces nausea. Not a treatment dose | 0.25mg/0.5mg pen |
| 2. First maintenance dose | 0.5mg | From week 5, at least 4 weeks | Standard treatment dose for many patients | 0.25mg/0.5mg pen |
| 3. Increased dose | 1mg | After at least 4 weeks on 0.5mg, if needed | More blood sugar control | 1mg pen |
| 4. Maximum dose | 2mg | After at least 4 weeks on 1mg, if needed | Highest approved dose | 2mg pen |
Do not take more than 2mg a week, and do not change your dose without your doctor’s advice.
How to take your weekly dose
- Inject on the same day each week, at any time of day, with or without food.
- Inject under the skin of your stomach, thigh or upper arm. Rotate sites each week.
- Use a new needle for every injection.
- You can change your injection day if your last dose was at least 2 days (48 hours) earlier.
which pen delivers which dose
Ozempic pens explained: 0.25mg/0.5mg, 1mg and 2mg
Each Ozempic pen is a pre-filled, multi-dose pen with a fixed dose selector. The pen you need depends on your current weekly dose, and one pen usually lasts about a month.
| Pen | Doses it can deliver | Who uses it | Available at DoctorOnCall |
| Ozempic 0.25mg/0.5mg pen | 0.25mg or 0.5mg | Patients starting treatment or maintained on 0.5mg | Yes |
| Ozempic 1mg pen | 1mg only | Patients stepped up to 1mg | Yes |
| Ozempic 2mg pen | 2mg only | Patients stepped up to the maximum dose | No |
When you start, you will typically use the 0.25mg/0.5mg pen for your first 4 weeks at 0.25mg, then continue on the same pen type at 0.5mg. Your doctor will prescribe the 1mg pen only if you move to 1mg.
Can I count clicks to take a different dose?
No. Some websites share “click charts” for drawing custom doses from a higher-strength pen. This is not an approved way to use Ozempic: the pen is designed to deliver only the doses shown on its counter, and partial doses may be inaccurate. If you need a different dose, ask your doctor to prescribe the correct pen.
Storing your pen
- Before first use: keep in the fridge at 2°C to 8°C. Do not freeze.
- After first use: keep for up to 56 days below 30°C or in the fridge, with the cap on.
- Throw away the pen after 56 days even if medicine is left.
Missed doses, side effects and dose changes
What if I miss an Ozempic dose?
- Within 5 days of the missed dose: take it as soon as you remember, then go back to your usual day.
- More than 5 days have passed: skip the missed dose and take the next one on your usual day.
- Never take two doses to make up for a missed one.
If you miss 2 or more weekly doses in a row, speak to your doctor before restarting. You may need to restart at a lower dose.
Side effects by dose
Stomach side effects are most common when you start or increase your dose, which is why Ozempic steps up gradually. They usually ease within a few weeks.
- Common: nausea, diarrhoea, vomiting, constipation, stomach pain, reduced appetite.
- Low blood sugar: more likely if you also take insulin or a sulphonylurea. Your doctor may adjust those medicines.
- Seek medical help urgently for severe, persistent stomach pain (possible pancreatitis), pain in the upper right abdomen or yellowing skin (possible gallbladder problems), or changes in vision.
Ozempic is not suitable if you or your family have a history of medullary thyroid cancer or MEN 2, or if you are pregnant or planning pregnancy. Tell your doctor about all your conditions and medicines.
When will my doctor change my dose?
Your doctor reviews your blood sugar (usually HbA1c) and side effects at each stage. They may keep you on 0.5mg if your levels are on target, increase to 1mg or 2mg if more control is needed, or keep you at a lower dose if side effects are hard to manage.
