Axcel Kaopec Suspension

Axcel Kaopec Suspension is an oral liquid suspension made by Kotra Pharma (M) Sdn Bhd in Malaysia under the Axcel brand. Each 5 ml contains 1 g of light kaolin and 20 mg of citrus pectin, with methylparaben 0.1% w/v included as a preservative. It is supplied as a 120 ml bottle and should be shaken well before every dose so that the suspended particles are evenly distributed.

Light kaolin is a purified natural clay, and pectin is a soluble fibre extracted from citrus fruit peel; together they are classified as intestinal adsorbents. Unlike some other stomach medicines, this suspension does not contain loperamide, an opioid-type agent, or any antibiotic. A key point for safe use is that kaolin and pectin can bind to other medicines in the gut, so the timing of other medicines around this suspension matters.

It is an over-the-counter product, so it can be purchased from pharmacies without a prescription. The Malaysian and Singapore product information state a dose for children aged 6 to 12 years and advise against use in children under 6 years, so parents and caregivers should read the label carefully. Do not give this suspension to children under 6 years of age. Take other medicines, especially antibiotics, at a different time from this suspension, generally leaving a gap of at least 2 hours unless a pharmacist advises otherwise, because kaolin and pectin can reduce their absorption. Drink plenty of fluids during any episode of loose stools.

If you miss a dose, skip it and take the next dose only when needed; do not double the dose. Store in a cool, dry place at the temperature stated on the packaging (usually below 30°C), away from direct sunlight, with the cap tightly closed. Do not freeze the suspension, and keep it out of the sight and reach of children.

• Common: Constipation, which is the most commonly reported effect of kaolin and pectin preparations.
• Common: Chalky taste or a feeling of fullness or bloating in the stomach.
• Serious: Severe constipation that may lead to faecal impaction, which may rarely occur in children and elderly patients and needs medical attention because it can cause intestinal blockage.
• Serious: Signs of dehydration or worsening abdominal pain, fever, or blood in the stool while taking the suspension, which requires prompt medical review because it may point to an underlying problem that a suspension alone cannot address.
• Serious: Allergic reaction such as rash, itching, or swelling of the face, which requires urgent care.

• Kaolin and pectin may interfere with the absorption of other medicines, including antibiotics, so take other medicines at a different time from this suspension.
• Medicines with a narrow margin of safety, such as digoxin, and medicines needing steady blood levels, such as antiepileptic and thyroid medicines, should be spaced well apart from this suspension and discussed with a pharmacist.
• No specific food interactions are known, but avoid taking the suspension at the same time as iron or mineral supplements.

• Shake the bottle vigorously before each dose so the kaolin and pectin are evenly mixed.
• Use a proper measuring cup instead of a kitchen spoon to get the dose right, especially for children.
• Leave at least 2 hours between this suspension and other medicines unless your pharmacist says otherwise.
• Drink water or oral rehydration fluids regularly during any bout of loose stools.
• Keep the cap tightly closed to prevent the suspension from drying out or becoming contaminated.
• Do not use it in a child under 6 years old, and check the age group on the label before giving it to anyone in the family.

Precautions: Axcel Kaopec Suspension is not recommended for children under 6 years of age. Take extra care in elderly people and children, in whom severe constipation may rarely lead to faecal impaction. Kaolin and pectin can reduce the absorption of other medicines, so keep a gap between doses and speak to a pharmacist if you take regular medication. When to See a Doctor or Pharmacist: See a doctor if loose stools last longer than 2 days, if there is fever, blood or mucus in the stool, severe abdominal pain, repeated vomiting, or if you are unable to keep fluids down. Seek urgent help for signs of dehydration, especially in young children, older people, or anyone who is unwell, and for any rash, swelling, or difficulty breathing after taking the suspension.
