Overview

Clozaril 100mg Tablet is a prescription-only medicine (POM) presented as a tablet, with Clozapine 100mg as its active ingredient content.

This page is limited to product, safety and handling information, in line with Malaysian rules on prescription medicine information for the public. Your prescriber or pharmacist is the right source for anything beyond that, together with the leaflet inside the pack.

Through DoctorOnCall, this product is supplied after an online consultation with a licensed doctor and a valid prescription, and it is checked by a registered pharmacist before dispensing. Take it exactly as prescribed, and speak to your doctor before stopping, changing the dose, or starting any other medicine alongside it.

Directions for Use

Take Clozaril 100mg Tablet exactly as instructed by your psychiatrist, at the specific times and doses set out in your titration schedule, and never adjust the dose, skip doses, or restart after a missed period yourself – restarting after an interruption of more than 48 hours requires re-titration from a low dose under your specialist’s direct guidance because of the risk of a severe drop in blood pressure or seizure.

Dosage

Clozaril 100mg Tablet is started at a low dose (typically 12.5mg) and increased gradually and slowly by your psychiatrist according to an individualised titration schedule, usually reaching a maintenance dose in the range of 300-450mg daily in divided doses. Treatment requires a baseline full blood count before starting, followed by mandatory regular blood monitoring (initially weekly, then less frequently as directed by your specialist) for as long as you remain on this medicine.

Storage Instructions

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

Contraindications

  • History of clozapine-induced agranulocytosis or severe neutropenia
  • Uncontrolled epilepsy
  • Severe heart disease, myocarditis, or paralytic ileus
  • Bone marrow disorders or concomitant use with other medicines that significantly suppress bone marrow function

Drug / Food Interactions

  • Carbamazepine and other bone-marrow-suppressing medicines are contraindicated due to a compounded risk of agranulocytosis
  • CYP1A2 inhibitors (e.g. fluvoxamine, ciprofloxacin) can significantly raise clozapine blood levels
  • Benzodiazepines increase the risk of severe sedation, breathing difficulty, and collapse when combined with clozapine
  • Starting or stopping smoking changes clozapine blood levels significantly (smoking lowers levels via CYP1A2 induction) and requires your psychiatrist to review your dose

Quick Tips

  • Never miss a scheduled blood test – regular full blood count monitoring is mandatory and non-negotiable for as long as you take this medicine
  • Contact your psychiatric care team immediately if you develop fever, sore throat, mouth ulcers, or any sign of infection
  • Never adjust your own dose or restart this medicine after any gap without your psychiatrist’s direct guidance
