Overview

Clozaril 25mg Tablet is a prescription-only medicine (POM) presented as a tablet, with Clozapine 25mg as its active ingredient content.

This page is limited to product, safety and handling information, in line with Malaysian rules on prescription medicine information for the public. Your prescriber or pharmacist is the right source for anything beyond that, together with the leaflet inside the pack.

Through DoctorOnCall, this product is supplied after an online consultation with a licensed doctor and a valid prescription, and it is checked by a registered pharmacist before dispensing. Take it exactly as prescribed, and speak to your doctor before stopping, changing the dose, or starting any other medicine alongside it.

Directions for Use

Clozaril tablets are taken orally as directed by the psychiatrist, usually starting once daily and increasing to a divided dosing schedule as titration progresses. Patients must attend all scheduled blood tests before each supply is dispensed, as required by the clozapine monitoring programme. If a dose is missed for more than 48 hours, treatment may need to be restarted at a lower dose under medical supervision rather than resumed at the previous dose – contact your care team promptly if doses have been missed.

Dosage

Clozaril is started at a low dose and increased gradually over several weeks under close specialist supervision, with the dose individualised based on response, tolerability, and blood monitoring results. Because of the risk of agranulocytosis and other serious effects, dosing, titration pace, and any dose changes must be determined solely by the treating psychiatrist as part of a registered clozapine monitoring programme – this medicine must never be started, stopped, or adjusted independently by the patient.

Storage Instructions

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

Side Effects

  • Drowsiness or dizziness
  • Increased heart rate
  • Weight gain
  • Constipation
  • Excessive salivation
  • Low blood pressure
  • Muscle stiffness
  • Dry mouth
  • Increased blood sugar levels
  • Risk of seizures (in higher doses)

Contraindications

  • Known hypersensitivity to clozapine or any component of the formulation
  • History of clozapine-induced agranulocytosis or severe neutropenia
  • Uncontrolled epilepsy
  • Severe cardiac disease, such as myocarditis or uncontrolled arrhythmias
  • Paralytic ileus or other severe bowel motility disorders
  • Concurrent use with other medicines known to cause significant bone marrow suppression

Drug / Food Interactions

  • Smoking cessation or reduction can raise clozapine blood levels significantly – inform your care team immediately if you stop or reduce smoking, as your dose may need urgent review
  • Combining with other medicines that suppress bone marrow function increases the risk of severe blood disorders and should be avoided unless specifically directed by your specialist
  • Alcohol and other sedating medicines can increase drowsiness and the risk of falls
  • Certain antibiotics, antifungals, and other medicines can raise or lower clozapine blood levels – always inform every prescriber that you are taking clozapine before starting any new medicine

Quick Tips

  • Never miss your scheduled blood tests – your continued supply of Clozaril depends on your monitoring results
  • Tell your care team immediately if you develop fever, sore throat, mouth ulcers, or signs of infection, as these can be early signs of agranulocytosis
  • Inform your psychiatrist immediately if your smoking habits change, as this can significantly affect your clozapine blood levels
  • Do not stop this medicine suddenly without medical advice, and never resume a missed dose schedule without checking with your care team first
