Cosmofer 100mg Solution for Injection

Cosmofer 100mg Solution for Injection is a prescription intravenous (and, in some settings, intramuscular) iron replacement medicine containing low molecular weight iron dextran, equivalent to 100mg of elemental iron per ampoule. It is used to treat iron deficiency in adults when oral iron supplements cannot be used, have not been tolerated, or are not effective enough to correct the deficiency, and when a rapid replenishment of iron stores is clinically required.

Because Cosmofer carries a risk of serious hypersensitivity reactions, including anaphylaxis, it must only be administered in a healthcare setting equipped to manage such reactions, by trained staff who can monitor the patient closely during and after each infusion or injection. Cosmofer is not a medicine that is self-administered at home, and its use, dose, and administration schedule are determined entirely by the treating doctor based on the calculated total iron requirement for each patient.

Cosmofer delivers iron directly into the bloodstream in a complex bound to dextran, which is designed to release iron gradually to the body’s iron storage and transport proteins (such as ferritin and transferrin). This replenishes iron stores that are needed for the production of haemoglobin, the protein in red blood cells that carries oxygen throughout the body.

Cosmofer is administered by slow intravenous injection, intravenous infusion (diluted in sodium chloride solution), or in some cases by deep intramuscular injection, only by trained healthcare staff in a setting where resuscitation facilities are available. Patients are monitored closely during administration and for a period afterwards for signs of an allergic or hypersensitivity reaction.

The total dose of Cosmofer required is calculated individually by the treating doctor based on the patient’s body weight and haemoglobin level, using a specific formula to determine the total iron deficit. This total dose may be given as a single infusion or divided into several sessions, depending on the clinical situation. A test dose is typically given before the first full dose to check for hypersensitivity reactions. Dosing must always be determined and administered by qualified healthcare professionals – this medicine is never self-administered or self-dosed.

Use this product before the expiry date stated on the packaging. Do not use the medicine if the expiry date has passed.

Store below 30°C in a dry place, away from direct sunlight and out of reach of children. Do not freeze.

• Headache or dizziness
• Nausea or metallic taste
• Injection or infusion site reactions, including pain, swelling, or discolouration of the skin
• Flushing or changes in blood pressure
• Hypersensitivity reactions, which can range from mild to severe (anaphylaxis) and require immediate medical attention

• Known hypersensitivity to iron dextran or any component of the formulation
• History of severe allergic disorders
• Iron overload conditions or disturbances in iron utilisation, such as haemochromatosis
• Decompensated liver disease
• Active acute or chronic infection

• Oral iron supplements should generally not be taken at the same time as Cosmofer – your doctor will advise when oral iron can be safely restarted
• May interfere with certain laboratory tests for serum iron and iron-binding capacity for a period after administration
• Inform your doctor of all other medicines and supplements you are taking before receiving this treatment

• Attend your infusion or injection appointment as scheduled, and arrange to stay at the healthcare facility for the recommended observation period afterwards
• Tell the healthcare team immediately if you notice itching, rash, difficulty breathing, or dizziness during or after administration
• Avoid taking oral iron supplements around the same time unless specifically advised by your doctor
• Keep your follow-up blood test appointments so your iron levels and response to treatment can be assessed
