Overview

Daxas 500mcg Tablet contains roflumilast 500mcg, a selective phosphodiesterase-4 (PDE4) inhibitor used under specialist supervision to reduce the risk of flare-ups (exacerbations) in adults with severe chronic obstructive pulmonary disease (COPD) associated with chronic bronchitis and a history of frequent exacerbations. It is not a bronchodilator and does not relieve sudden breathing difficulty.

Roflumilast works by reducing lung inflammation at a cellular level rather than opening the airways directly, so it is taken alongside – never in place of – your regular COPD inhalers and bronchodilators as prescribed by your specialist.

How it Works

Roflumilast inhibits the PDE4 enzyme, increasing intracellular cyclic AMP within inflammatory and structural lung cells. This reduces the release of inflammatory mediators associated with COPD, which over time lowers the frequency of exacerbations, but this anti-inflammatory action does not provide immediate bronchodilation.

Directions for Use

Take Daxas 500mcg Tablet at the same time each day, with or without food, and continue all your other prescribed COPD medicines and inhalers exactly as directed. Do not stop any of your regular COPD treatments without your specialist’s advice, even if your breathing feels stable.

Dosage

The usual dose of Daxas 500mcg Tablet is one tablet once daily, taken long-term as a maintenance treatment under the supervision of a respiratory specialist. It is not used for sudden worsening of breathing and does not replace your rescue inhaler or other prescribed COPD medicines.

Drug / Food Interactions

  • Strong enzyme-inhibiting medicines such as ketoconazole and fluvoxamine can increase roflumilast exposure and side effects
  • Theophylline should be combined only with caution and close monitoring, as data on this combination are limited
  • Certain hormonal contraceptives (e.g. containing gestodene and ethinylestradiol) may have altered levels when combined

Quick Tips

  • Always continue your other prescribed COPD medicines and inhalers – this medicine works alongside them, not instead of them
  • Weigh yourself periodically and tell your specialist about any significant, unintended weight loss
  • Report any new mood changes, anxiety, or feelings of hopelessness to your doctor immediately
  • Do not use this medicine for sudden breathlessness – use your rescue inhaler and seek medical help as advised

Frequently Asked Questions

Is Daxas 500mcg Tablet a reliever inhaler for sudden breathlessness?

No, roflumilast is not a bronchodilator and does not act quickly. It is a long-term maintenance treatment used alongside your regular inhalers to reduce how often COPD flare-ups occur.

Why does my specialist need to monitor my mood while I take Daxas 500mcg Tablet?

Roflumilast has been associated with psychiatric side effects such as anxiety, low mood, and, rarely, suicidal thoughts in some patients, so any new or worsening mood symptoms should be reported and reviewed promptly.

Will Daxas 500mcg Tablet cause weight loss?

Some patients experience decreased appetite and weight loss while taking roflumilast. Your specialist will monitor your weight during treatment, and you should report any significant or unintended weight loss.

How soon will I notice fewer COPD flare-ups?

The benefit of roflumilast in reducing exacerbations builds up over months of consistent use as part of your overall COPD management plan, rather than providing an immediate effect.

Can I stop my other COPD inhalers once I start Daxas 500mcg Tablet?

No, you must continue all other prescribed COPD medicines and inhalers exactly as directed by your specialist; roflumilast is an add-on treatment, not a replacement.
