Overview

Firialta 10mg Tablet contains finerenone, a non-steroidal mineralocorticoid receptor antagonist (MRA) used in adults with chronic kidney disease (CKD) associated with type 2 diabetes, to reduce the risk of kidney function decline, kidney failure, and cardiovascular events such as heart attack.

It works by blocking the mineralocorticoid receptor, which when overactivated contributes to inflammation and scarring (fibrosis) in the kidneys and heart. By blocking this receptor, finerenone helps protect kidney and cardiovascular function over time.

How it Works

Firialta selectively blocks the mineralocorticoid receptor in the kidneys and heart, reducing overactivation that would otherwise drive inflammation and fibrosis, and helping to slow the progression of kidney damage associated with diabetes.

Directions for Use

Take the tablet once daily, with or without food, at about the same time each day; swallow it whole or, if needed, it may be crushed and mixed with water or soft food immediately before use, as advised by your doctor.

Dosage

The starting dose depends on the patient’s kidney function (estimated GFR) and blood potassium level at baseline, and is individualised and titrated by the treating doctor, who monitors blood potassium and kidney function periodically during treatment.

Side Effects

  • High blood potassium (hyperkalaemia) – this is the most important side effect to monitor for
  • Low blood pressure (hypotension)
  • Hyponatraemia (low blood sodium)
  • Generalised oedema (swelling)

Contraindications

  • Known hypersensitivity to finerenone or any ingredient in the tablet
  • Adrenal insufficiency
  • Concurrent use with strong CYP3A4 inhibitors
  • Severe liver impairment

Drug / Food Interactions

  • Potassium supplements and potassium-sparing diuretics can further raise blood potassium and are generally avoided unless closely monitored by your doctor.
  • Strong CYP3A4 inhibitors (such as certain antifungals and some antibiotics) can significantly raise finerenone levels and are usually contraindicated.
  • ACE inhibitors, ARBs, and other medicines that raise potassium require closer monitoring when combined with finerenone.
