Overview

Isopto Carpine 2% Eye Drop contains pilocarpine 2%, a direct-acting cholinergic (miotic) eye drop used to lower elevated intraocular pressure in glaucoma, and sometimes to constrict the pupil after an eye examination or procedure. It is manufactured by Alcon.

Pilocarpine works by directly stimulating muscarinic receptors in the eye, causing the pupil to constrict (miosis) and the ciliary muscle to contract. This pulls open the drainage channels of the eye, allowing aqueous humour to drain more easily and lowering the pressure inside the eye.

How it Works

Pilocarpine directly activates muscarinic acetylcholine receptors on the iris sphincter and ciliary muscle. Contraction of these muscles constricts the pupil and pulls open the trabecular meshwork, the eye’s main drainage pathway, improving the outflow of aqueous humour and reducing intraocular pressure.

Directions for Use

Wash your hands before use. Tilt your head back, gently pull down the lower eyelid to form a small pocket, and instil one drop without letting the dropper tip touch the eye or eyelid. Close your eye and press gently on the inner corner near the nose for about a minute. Expect some temporary blurring of vision and dim-light adjustment difficulty after each dose, particularly at first.

Dosage

The usual dose of Isopto Carpine 2% Eye Drop is one drop in the affected eye(s), typically several times a day as directed by your doctor, since the effect of each drop wears off after some hours. Your doctor will confirm the exact frequency and which eye(s) to treat, and will monitor your intraocular pressure to guide any dose changes.

  • Dizziness
  • Nausea
  • Diarrhoea

Contraindications

  • Known hypersensitivity to pilocarpine or any ingredient in this product
  • Acute anterior uveitis (active inflammation inside the eye) unless directed otherwise by an ophthalmologist
  • Conditions where pupil constriction is undesirable, such as certain forms of secondary glaucoma

Drug / Food Interactions

  • Other eye drops used at the same time should be given at least 5 minutes apart to avoid one washing out the other
  • Systemic anticholinergic medicines (e.g. certain antihistamines, some antidepressants) may reduce the pupil-constricting effect of pilocarpine
  • Other cholinergic or anticholinesterase medicines may add to pilocarpine’s effects and increase side effects

Quick Tips

  • Use exactly as often as your doctor prescribes, as the effect wears off within hours
  • Be extra cautious with night-time activities such as driving, as vision in dim light is often reduced
  • Do not let the dropper tip touch your eye or eyelid to avoid contamination
  • Wait at least 5 minutes between this and any other eye drop
  • Attend all follow-up eye pressure checks, as glaucoma often has no early symptoms
  • Seek urgent eye care for any sudden flashes, floaters, or loss of vision

Frequently Asked Questions

What is Isopto Carpine 2% Eye Drop used for?

Isopto Carpine 2% Eye Drop is used to lower raised pressure inside the eye in glaucoma, and is sometimes used to constrict the pupil after certain eye examinations or procedures.

Why does my vision become blurry after using this eye drop?

Pilocarpine constricts the pupil, which naturally causes some temporary blurring and reduced vision in dim light. This effect usually lessens as your eyes adjust to treatment.

Can I drive at night while using this medicine?

You should be cautious with night driving or other low-light activities, as pilocarpine can significantly reduce your ability to see in dim light.

What should I do if I miss a dose of Isopto Carpine 2% Eye Drop?

Apply the missed drop as soon as you remember. If it is almost time for your next dose, skip the missed one and continue your regular schedule – do not apply a double dose.

How often do I need to use this eye drop?

Because the effect wears off after some hours, pilocarpine is usually used multiple times a day. Follow your doctor’s exact schedule for consistent eye pressure control.
