Mounjaro 7.5mg/dose KwikPen Solution for Injection in Pre-filled Pen

Mounjaro 7.5mg/dose KwikPen is a prescription injectable medicine containing tirzepatide, delivered as a solution for injection in a pre-filled pen. Tirzepatide is a dual GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptor agonist manufactured by Eli Lilly. It is registered with the National Pharmaceutical Regulatory Agency (NPRA) in Malaysia and is used, together with diet and exercise, to improve blood glucose control in adults with type 2 diabetes mellitus.

Each Mounjaro KwikPen is a single-use, pre-set pen designed for once-weekly self-injection, and the 7.5mg dose represents one step in a gradual dose-titration schedule that starts at a lower strength and is increased over time under a doctor’s guidance. Because tirzepatide is a relatively new class of medicine in Malaysia, patients should only start or adjust treatment following an appropriate medical assessment, and Mounjaro should not be used as a substitute for insulin in patients who require it.

Tirzepatide activates both the GIP and GLP-1 receptors on pancreatic beta cells, gut, and brain. This dual action increases insulin release when blood glucose is high, reduces glucagon secretion, slows gastric emptying, and acts on appetite centres in the brain to reduce hunger. Together, these effects lower blood glucose levels and are also associated with reduced food intake and body weight.

Mounjaro KwikPen is injected subcutaneously (under the skin) into the abdomen, thigh, or upper arm, rotating the injection site with each dose. Each pen is for single use only and should be disposed of after one injection, even if some solution remains. Patients should be trained by a healthcare professional on correct injection technique before using the pen independently. If a dose is missed and it has been less than 4 days since the missed dose, it should be taken as soon as possible; if more than 4 days have passed, the missed dose should be skipped and the next dose taken on the regular scheduled day – do not inject two doses close together.

Mounjaro is usually started at a low dose and increased gradually every 4 weeks according to the patient’s response and tolerability, up to a maximum recommended dose. The 7.5mg/dose KwikPen is injected once weekly, on the same day each week, at any time of day, with or without food. The exact starting dose, titration schedule, and target maintenance dose must be determined and adjusted by the prescribing doctor based on individual blood glucose control and tolerability – patients should never change their dose or switch strengths without medical advice.

Use this product before the expiry date stated on the packaging. Do not use the medicine if the expiry date has passed.

Store unused Mounjaro KwikPens in a refrigerator (2°C to 8°C); do not freeze, and discard the pen if it has been frozen. Once in use, a pen may be kept below 30°C or in a refrigerator for up to 21 days, away from direct light and heat, and out of reach of children. Always keep the pen cap on when not in use.

• Known hypersensitivity to tirzepatide or any component of the formulation
• Personal or family history of medullary thyroid carcinoma
• Patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
• Not recommended for use during pregnancy or breastfeeding unless specifically advised by a doctor

• May increase the risk of low blood glucose (hypoglycaemia) when combined with insulin or sulfonylureas – dose adjustment of these medicines may be needed
• Can delay gastric emptying, which may affect the absorption of oral medicines taken at the same time
• May reduce the effectiveness of oral contraceptives for a period after starting or increasing the dose – discuss alternative or additional contraception with your doctor
• Always inform your doctor of all prescription medicines, over-the-counter products, and supplements you are taking before starting Mounjaro

• Inject on the same day each week to help maintain a consistent routine
• Rotate injection sites to reduce the risk of skin reactions
• Eating smaller, slower meals in the first few weeks may help reduce nausea as your body adjusts
• Seek medical attention promptly if you develop severe abdominal pain, as this can occasionally signal pancreatitis
