Overview

Pulmicort 500mcg/ml Respule contains budesonide 500mcg/mL (0.5mg/mL), an inhaled corticosteroid supplied as a single-use nebule (respule) of liquid suspension for nebulised inhalation. It is used for the long-term control of asthma, particularly in patients (including young children) who have difficulty using a metered-dose inhaler or dry powder inhaler correctly.

Budesonide works directly in the airway lining to reduce inflammation, swelling, and mucus production associated with asthma. Regular use over time reduces airway sensitivity and the frequency of asthma flare-ups, though it does not relieve a sudden asthma attack in progress.

How it Works

Budesonide is a corticosteroid that, once deposited in the airway lining by nebulised inhalation, binds to glucocorticoid receptors in airway cells. This switches off the production of inflammatory chemicals and reduces swelling, mucus secretion, and the sensitivity of the airways to asthma triggers, calming inflammation over time rather than providing immediate bronchodilation.

Directions for Use

Shake the respule gently, then open and empty its full contents into the nebuliser chamber of a suitable jet nebuliser (not an ultrasonic nebuliser, which can inactivate budesonide); it should not be mixed with other medicines in the same nebule unless your doctor or pharmacist has specifically advised this. Breathe the mist in through a mouthpiece or well-fitted face mask as instructed, and rinse the mouth (and wash the face if a mask was used) with water afterwards to reduce the risk of oral thrush and skin irritation.

Dosage

The dose of Pulmicort 500mcg/ml Respule is individualised by your doctor based on age, asthma severity, and response to treatment, and is usually given once or twice daily via a jet nebuliser. Your doctor will advise the correct dose, frequency, and how to adjust it during periods of worsened symptoms – do not change the dose on your own.

Shelf Life

Use this product before the expiry date stated on the packaging. Do not use the medicine if the expiry date has passed.

Storage Instructions

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

Side Effects

  • Headache
  • Nausea or upset stomach
  • Throat irritation (with inhaled forms)
  • Cough or hoarseness
  • White patches in the mouth (oral thrush)
  • Increased risk of infections
  • Dizziness
  • Acne or skin thinning (with prolonged use)

Contraindications

  • Known hypersensitivity to budesonide or any ingredient in this product
  • Active, untreated respiratory tract infections such as tuberculosis or fungal/viral lung infections, unless specifically directed by a doctor

Drug / Food Interactions

  • Ketoconazole, itraconazole, ritonavir, and other strong CYP3A4 inhibitors can significantly increase systemic budesonide exposure
  • Other corticosteroids (oral, injected, or inhaled) taken together may increase the total steroid load and side-effect risk
  • Grapefruit juice may modestly increase budesonide absorption

Quick Tips

  • Rinse your mouth with water (and spit it out) after every nebulisation to prevent oral thrush
  • Wash and dry the face if a mask is used, to reduce skin irritation
  • Use only with a jet nebuliser, not an ultrasonic device, unless advised otherwise
  • Continue regular use even when asthma is well controlled – this medicine is for prevention, not for relieving a sudden attack
  • Keep a fast-acting reliever inhaler available for sudden breathing difficulty, as advised by your doctor
  • Clean the nebuliser equipment as instructed to prevent contamination

Frequently Asked Questions

What is Pulmicort 500mcg/ml Respule used for?

Pulmicort 500mcg/ml Respule contains budesonide, an inhaled corticosteroid delivered via nebuliser for the long-term control and prevention of asthma symptoms, particularly in patients who cannot use a standard inhaler.

Can I use this to relieve a sudden asthma attack?

No, this is a preventer medicine that reduces airway inflammation over time – it does not act quickly enough to relieve a sudden attack. Use your prescribed fast-acting reliever inhaler for sudden symptoms and seek medical help if breathing difficulty is severe.

Why do I need to rinse my mouth after each dose?

Rinsing the mouth (and gargling, then spitting out the water) after each nebulisation removes leftover medicine from the mouth and throat, which significantly reduces the risk of developing oral thrush and hoarseness.

Can I use an ultrasonic nebuliser instead of a jet nebuliser?

No, ultrasonic nebulisers can break down and inactivate the budesonide suspension. Use only the type of nebuliser recommended by your doctor or pharmacist.

Is it safe to use this medicine for young children?

Nebulised budesonide is commonly used in children when prescribed by a doctor, as it does not require the coordination needed for a metered-dose inhaler; the dose and mask fit should always be guided by your child’s doctor.
