Overview

Ranexa 375mg Tablet contains ranolazine 375mg in an extended-release tablet, an anti-anginal medicine used to treat chronic stable angina (recurring chest pain due to reduced blood flow to the heart), usually as add-on therapy when other angina medicines have not fully controlled symptoms.

Ranolazine works differently from most other angina medicines: it inhibits the late inward sodium current in heart muscle cells, which reduces excess calcium build-up inside the cells. This improves the heart muscle’s ability to relax between beats and reduces the mismatch between oxygen supply and demand that causes angina pain, without significantly changing your heart rate or blood pressure.

How it Works

Ranolazine selectively inhibits the late inward sodium current (late INa) in heart muscle cells. By reducing sodium entry, it lowers the resulting build-up of intracellular calcium, which improves diastolic (relaxation-phase) function of the heart muscle and reduces the oxygen demand that triggers angina, without the heart-rate- or blood-pressure-lowering effects typical of other anti-anginal drug classes.

Directions for Use

Take Ranexa 375mg Tablet with or without food, but be consistent with how you take it each time. Swallow the tablet whole – do not crush, chew, or split it, as this is an extended-release formulation designed to release the medicine slowly over the day. Avoid grapefruit juice while taking this medicine.

Dosage

Ranexa 375mg Tablet is usually taken twice daily, with your doctor typically starting at a lower dose and adjusting the strength over time based on your symptom control and tolerability, up to a maximum recommended dose. This tablet strength reflects one step in that individualised titration.

Shelf Life

Use this product before the expiry date stated on the packaging. Do not use the medicine if the expiry date has passed.

Storage Instructions

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

Side Effects

  • Dizziness
  • Constipation
  • Nausea
  • Headache
  • Fatigue
  • Dry mouth
  • Palpitations
  • Abdominal discomfort

Contraindications

  • Known hypersensitivity to ranolazine or any ingredient in this product
  • Clinically significant liver impairment
  • Concomitant use with strong CYP3A4 inhibitors (e.g. ketoconazole, clarithromycin, certain HIV protease inhibitors) or strong CYP3A4 inducers (e.g. rifampicin, phenytoin, carbamazepine, St John’s Wort)
  • Pre-existing prolongation of the QT interval on ECG

Drug / Food Interactions

  • Strong or moderate CYP3A4 inhibitors significantly raise ranolazine levels and increase the risk of side effects – some combinations are contraindicated
  • Ranolazine increases blood levels of digoxin and simvastatin; your doctor may need to limit your simvastatin dose or monitor digoxin levels more closely
  • Grapefruit juice inhibits the same enzyme that clears ranolazine and can raise its levels – avoid grapefruit juice and grapefruit products
  • Other medicines that prolong the QT interval may add to the risk of heart rhythm disturbances

Quick Tips

  • Swallow the tablet whole – never crush, chew, or split it
  • Avoid grapefruit juice and grapefruit products throughout treatment
  • Report palpitations, a fast or irregular heartbeat, or fainting to your doctor promptly
  • Continue taking your other prescribed angina medicines unless your doctor tells you to stop
  • Keep to your doctor’s plan for a fast-acting nitrate (if prescribed) for sudden chest pain, as this medicine is not for immediate relief of an angina attack

Frequently Asked Questions

What is Ranexa 375mg Tablet used for?

Ranexa 375mg Tablet is used to treat chronic stable angina, usually as an add-on medicine when angina is not fully controlled by first-line treatments such as beta-blockers or calcium channel blockers.

Can I use this medicine to relieve sudden chest pain?

No. This is a maintenance medicine taken regularly to reduce how often angina occurs – it does not act quickly enough to relieve a sudden attack. Continue using any fast-acting nitrate your doctor has prescribed for sudden chest pain.

Why must I avoid grapefruit juice with this medicine?

Grapefruit juice blocks the same liver enzyme (CYP3A4) that breaks down ranolazine, which can raise ranolazine levels in your blood and increase the risk of side effects, including heart rhythm changes.

Why do the different Ranexa tablet strengths exist?

Ranolazine is usually started at a lower dose and increased gradually based on how well your angina is controlled and how well you tolerate the medicine – the different strengths allow your doctor to titrate your dose safely.

What should I do if I miss a dose of Ranexa 375mg Tablet?

Take the missed dose as soon as you remember, unless it is almost time for your next dose – in that case, skip the missed dose and continue your regular schedule. Do not double the dose.
