Overview

Tolura 40mg Tablet contains telmisartan, an angiotensin II receptor blocker (ARB) used to treat high blood pressure and to reduce cardiovascular risk in certain high-risk patients.

Telmisartan works by blocking the action of angiotensin II, a hormone that narrows blood vessels; by blocking this effect, blood vessels relax and widen, which lowers blood pressure and reduces strain on the heart.

How it Works

Tolura selectively blocks the AT1 receptor that angiotensin II binds to on blood vessel walls, preventing vasoconstriction and the release of aldosterone; this results in relaxed, widened blood vessels and reduced fluid retention, lowering blood pressure.

Directions for Use

Take at the same time each day, with or without food, and swallow the tablet whole with water. Do not stop taking this medicine without consulting your doctor, even if your blood pressure appears well controlled.

Dosage

The usual dose is one tablet once daily, which your doctor may adjust based on your blood pressure response; it may be taken with or without food.

Shelf Life

Refer to the expiry date printed on the packaging.

Storage Instructions

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

  • Muscle pain or back pain
  • Increased potassium levels (hyperkalemia)
  • Upper respiratory infections
  • – Skin rash or itching

  • Pregnancy, as ARBs can cause fetal harm
  • Severe liver impairment or biliary obstruction
  • Concurrent use with aliskiren in patients with diabetes

Drug / Food Interactions

  • Potassium supplements or potassium-sparing diuretics: combined use can raise blood potassium to dangerous levels.
  • NSAIDs (such as ibuprofen or diclofenac): may reduce the blood-pressure-lowering effect and increase the risk of kidney problems, especially in older adults or those with reduced kidney function.
  • Lithium: telmisartan can raise lithium blood levels, increasing the risk of lithium toxicity.
  • Other blood pressure medicines: may cause an additive drop in blood pressure.
  • Inform your doctor of all medicines you are taking, including over-the-counter and herbal products.

Quick Tips

  • Tell your doctor immediately if you become pregnant or are planning pregnancy, as this medicine must be stopped and switched to a pregnancy-safe alternative.
  • Have your blood pressure and kidney function checked regularly as advised by your doctor.
  • Avoid excessive use of over-the-counter painkillers such as ibuprofen without checking with your pharmacist, as these can reduce the medicine’s effectiveness.
  • Rise slowly from sitting or lying down if you feel dizzy, especially when starting treatment.
