Rybelsus 3mg vs 7mg vs 14mg: Dosage Guide & Which Strength to Order

Rybelsus 3mg, 7mg and 14mg at a glance
Rybelsus is taken in steps: 3mg for the first month, then 7mg, then 14mg if your blood sugar needs more control. The 3mg tablet is a starting dose only. It helps your stomach adjust and is not meant to control blood sugar on its own.
| Strength | Role | Typical duration | Who it is for |
|---|---|---|---|
| 3mg | Starting dose | First 30 days | Everyone beginning Rybelsus |
| 7mg | Maintenance dose | At least 30 days, often long term | Patients who have completed 30 days on 3mg |
| 14mg | Maximum dose | Long term | Patients who need further HbA1c reduction after 30 days or more on 7mg |
Your doctor decides when to move up. Do not change strength without a prescription review.
What is Rybelsus?
Rybelsus is the first oral GLP-1 receptor agonist. Its active ingredient is semaglutide, the same molecule used in Ozempic injections, in a once-daily tablet.
It is prescribed for adults with type 2 diabetes, alongside diet and exercise, to improve blood sugar control. It works by helping the pancreas release insulin when glucose is high, lowering the liver’s glucose output and slowing stomach emptying.
Rybelsus is a prescription-only medicine. It is not approved for weight loss, although some patients lose weight while taking it. It is not used for type 1 diabetes or diabetic ketoacidosis.
Rybelsus 3mg: the starting dose
Every patient starts on Rybelsus 3mg once daily for 30 days. This lead-in period lets your digestive system adjust and lowers the chance of nausea, vomiting and diarrhoea, the most common side effects of GLP-1 medicines.
The 3mg dose is not an effective long-term dose for blood sugar control. If you stay on 3mg beyond the first month without a doctor’s reason, you are unlikely to get the full benefit.
- Duration: 30 days
- Pack to order: one month’s supply of 3mg, or the [Rybelsus 3mg starter bundle]
- Next step: review with your doctor before day 30 to move to 7mg
Rybelsus 7mg: the maintenance dose
After 30 days on 3mg, the dose increases to 7mg once daily. For many patients, 7mg is the long-term dose that keeps HbA1c on target.
In the PIONEER 1 clinical trial, 7mg lowered HbA1c by about 1.2 percentage points over 26 weeks in adults with type 2 diabetes. Your doctor will usually check your HbA1c after at least 30 days on 7mg, and typically again at around three months.
- Duration: at least 30 days, often ongoing
- Who stays on 7mg: patients who reach their HbA1c target at this dose
- Next step: move to 14mg only if your doctor finds more control is needed
Rybelsus 14mg: the maximum dose
Rybelsus 14mg once daily is the highest approved dose for type 2 diabetes. It is used when HbA1c is still above target after at least 30 days on 7mg.
In PIONEER 1, 14mg lowered HbA1c by about 1.4 percentage points over 26 weeks, the largest reduction of the three strengths. Patients on 14mg also saw greater weight reduction than on lower doses, though Rybelsus is prescribed for blood sugar, not weight.
Use a single 14mg tablet. Taking two 7mg tablets to make 14mg has not been studied and is not recommended.
- Duration: long term
- Who moves to 14mg: patients who need further HbA1c reduction on 7mg
- Next step: ongoing HbA1c checks every three to six months
How to take Rybelsus correctly
Rybelsus only works if it is absorbed properly, and food, drinks and other tablets block that absorption. The rules are the same for 3mg, 7mg and 14mg.
- Take it first thing in the morning on an empty stomach.
- Swallow one tablet whole with no more than 120ml (half a glass) of plain water.
- Do not split, crush or chew the tablet.
- Wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.
- Keep tablets in the original blister pack until use, to protect them from moisture.
Waiting longer than 30 minutes can improve absorption further. Waiting less than 30 minutes reduces how much semaglutide your body takes in.
Missed a dose? Skip it and take your normal dose the next morning. Do not take two tablets to make up for it.
Which Rybelsus strength should you order?
Order the strength on your current prescription. Your stage of treatment decides which one that is.

| New to Rybelsus | 3mg, 30-day supply | [Rybelsus 3mg] or [3mg starter bundle] |
| Finished 30 days on 3mg, doctor has stepped you up | 7mg | [Rybelsus 7mg] |
| On 7mg and HbA1c is on target | 7mg (repeat) | [Rybelsus 7mg] |
| On 7mg, doctor has increased your dose | 14mg | [Rybelsus 14mg] |

Side effects and safety
Most side effects are digestive and are most common when starting or increasing the dose. They usually ease within a few weeks.
Common side effects: nausea, diarrhoea, vomiting, constipation, stomach pain, bloating and reduced appetite.
Seek medical help urgently if you have:
- Severe, persistent stomach pain that may spread to your back (possible pancreatitis)
- A lump or swelling in the neck, trouble swallowing or a hoarse voice
- Signs of dehydration from ongoing vomiting or diarrhoea
- Changes in vision
Tell your doctor before starting if you:
- Have a personal or family history of medullary thyroid cancer or MEN 2 syndrome
- Have had pancreatitis or diabetic eye disease
- Take insulin or a sulfonylurea, as the risk of low blood sugar increases
- Take levothyroxine, as Rybelsus can raise its levels
- Are pregnant, planning pregnancy or breastfeeding. Rybelsus should be stopped at least two months before a planned pregnancy.
Frequently asked questions
Can I start Rybelsus on 7mg? No. Everyone starts on 3mg for 30 days. Starting higher increases the risk of nausea and vomiting.
How long should I stay on Rybelsus 3mg? 30 days. After that, your doctor will usually move you to 7mg, because 3mg alone does not give effective blood sugar control.
Can I take two 7mg tablets instead of one 14mg tablet? No. This has not been studied and may not give the same effect. Use a single 14mg tablet.
Can I take Rybelsus at night? It is designed to be taken in the morning, on an empty stomach, at least 30 minutes before your first food, drink or other medicine of the day.
Can I drink coffee after taking Rybelsus? Wait at least 30 minutes. Coffee, tea and juice all reduce absorption if taken too soon.
Is Rybelsus the same as Ozempic? Both contain semaglutide. Rybelsus is a daily tablet; Ozempic is a weekly injection. The doses are not interchangeable, so switching needs a doctor’s guidance.
Do I need a prescription to buy Rybelsus online? Yes. Rybelsus is a prescription-only medicine. You can get a prescription through a DoctorOnCall online consultation.
