Rybelsus vs Ozempic: Semaglutide Tablet vs Weekly Injection

Rybelsus and Ozempic contain the same active ingredient, semaglutide, and both are prescribed for adults with type 2 diabetes. The difference is how you take it: Rybelsus is a tablet swallowed once a day, while Ozempic is a pre-filled pen injected once a week.
That single difference shapes everything else – the daily routine, how much semaglutide reaches the bloodstream, the doses available and the monthly cost. This guide compares both side by side so you can have a more informed discussion with your doctor.
Both medicines are prescription-only in Malaysia. A licensed doctor must assess whether either is suitable for you.

How each is taken: tablet vs pen
Rybelsus – one tablet every morning
Rybelsus must be taken in a specific way, because food and drink reduce how much semaglutide the stomach absorbs.
- Take it first thing in the morning, on an empty stomach.
- Swallow the tablet whole with no more than 120 ml (half a glass) of plain water.
- Wait at least 30 minutes before eating, drinking or taking other oral medicines.
Treatment usually starts at 3 mg daily for 30 days. This is a starter dose to help the stomach adjust, not a treatment dose. The doctor then increases it to 7 mg, and to 14 mg after at least another 30 days if more glucose control is needed.
Ozempic – one injection a week
Ozempic is injected under the skin of the abdomen, thigh or upper arm on the same day each week. It can be taken at any time of day, with or without meals.
- Weeks 1–4: 0.25 mg once weekly (starter dose).
- Week 5 onwards: 0.5 mg once weekly.
- If needed, after at least 4 weeks: 1 mg once weekly.
The pen uses a fine needle, and each dose takes a few seconds. A missed dose can be taken within 5 days; after that, skip it and resume on the usual day.
Which routine is easier?
Rybelsus suits people who prefer to avoid needles and can keep a fixed morning routine. Ozempic suits people who find a daily fasting window difficult, or who already take several tablets each morning. A weekly dose also means 52 doses a year instead of 365.
Results: blood sugar and body weight
Both medicines lower HbA1c (average blood sugar over about three months) by a similar range at their highest doses. Rybelsus and Ozempic have not been compared directly in a head-to-head trial, so the figures below come from separate placebo-controlled studies and should be read as a guide only.
| Trial (duration) | Dose | Average HbA1c reduction | Average weight change |
|---|---|---|---|
| PIONEER 1, Rybelsus (26 weeks) | 7 mg daily | about 1.2% | about −2.3 kg |
| PIONEER 1, Rybelsus (26 weeks) | 14 mg daily | about 1.4% | about −3.7 kg |
| SUSTAIN 1, Ozempic (30 weeks) | 0.5 mg weekly | about 1.5% | about −3.7 kg |
| SUSTAIN 1, Ozempic (30 weeks) | 1 mg weekly | about 1.6% | about −4.5 kg |
Why the pen reaches higher exposure. Only a small fraction of a semaglutide tablet is absorbed through the stomach. That is why the tablet dose is measured in milligrams per day, while the injected dose is a fraction of a milligram per week. Rybelsus 14 mg daily and Ozempic 0.5 mg weekly give broadly comparable results.
Weight reduction was a secondary finding in these diabetes trials. Ozempic and Rybelsus are not approved for weight management in Malaysia. For weight management, doctors may consider a different semaglutide product, Wegovy, which is registered for that use.
Side effects and safety
Because the active ingredient is the same, Rybelsus and Ozempic share the same side effect profile. Most effects are digestive, are strongest in the first weeks or after a dose increase, and ease over time.
Common side effects (both)
- Nausea, vomiting or diarrhoea
- Constipation, bloating or abdominal discomfort
- Reduced appetite
- Low blood sugar, mainly when combined with insulin or a sulfonylurea
Specific to the form
- Ozempic: redness or mild discomfort at the injection site
- Rybelsus: results depend on taking the tablet exactly as directed; a skipped fasting window lowers absorption
Not suitable for people with
- A personal or family history of medullary thyroid carcinoma (MTC)
- Multiple endocrine neoplasia syndrome type 2 (MEN 2)
- Type 1 diabetes or diabetic ketoacidosis
- Pregnancy or plans to become pregnant
Seek medical care promptly for severe, persistent abdominal pain (a possible sign of pancreatitis), signs of an allergic reaction, or sudden changes in vision. Tell the doctor about a history of pancreatitis, kidney problems or diabetic eye disease before starting either medicine.
Rybelsus vs Ozempic price in Malaysia
At maintenance doses, Rybelsus costs about RM230 less per month than Ozempic when bought as a 3-month supply at DoctorOnCall.
| Product | Pack | Lasts | Price | Per month |
|---|---|---|---|---|
| Rybelsus 3 mg | 30 tablets | 30 days | RM599 | RM599 |
| Rybelsus 3 mg | 30 tablets × 3 | 90 days | RM1,747 | RM582 |
| Rybelsus 7 mg or 14 mg | 30 tablets | 30 days | RM799 | RM799 |
| Rybelsus 7 mg or 14 mg | 30 tablets × 3 | 90 days | RM2,297 | RM766 |
| Ozempic 0.25 mg / 0.5 mg | 1 pen | 4 weeks at 0.5 mg | RM1,049 | RM1,049 |
| Ozempic 0.25 mg / 0.5 mg | 3 pens | 12 weeks at 0.5 mg | RM2,997 | RM999 |
| Ozempic 1 mg | 1 pen | 4 weeks | RM1,049 | RM1,049 |
| Ozempic 1 mg | 3 pens | 12 weeks | RM2,997 | RM999 |
The Rybelsus price is the same for 7 mg and 14 mg, and Ozempic costs the same at 0.5 mg and 1 mg, so moving up a dose does not raise the monthly cost of either. During the Ozempic starter phase, one 0.25 mg / 0.5 mg pen covers about 6 weeks.
Instalment plans are available on both ranges at checkout. Prices are correct as of 16 October 2026 and may change.
Which is better: Rybelsus or Ozempic?
Neither is better for everyone. Both work in the same way and give broadly similar blood sugar control at their higher doses; the right choice depends on routine, preference and medical history.
| Rybelsus may suit you if you | Ozempic may suit you if you |
|---|---|
| Prefer not to use needles | Are comfortable with a quick weekly injection |
| Can keep a fixed fasting window every morning | Find a daily 30-minute wait before breakfast difficult |
| Want a lower monthly cost | Take several other oral medicines in the morning |
| Have a regular, predictable morning routine | Want fewer doses to remember (52 a year, not 365) |
Switching between the two is possible but must be guided by a doctor, as the doses are not interchangeable milligram for milligram.
Frequently asked questions
Which is better, Ozempic or Rybelsus?
Neither is better for everyone. Both contain semaglutide and lower HbA1c by a similar amount at higher doses. Ozempic is a weekly injection; Rybelsus is a daily tablet with strict fasting rules. Your doctor will recommend one based on your health and routine.
Is Rybelsus cheaper than Ozempic?
Yes. At DoctorOnCall, Rybelsus 7 mg or 14 mg costs RM766 per month on a 3-month supply, compared with RM999 per month for Ozempic.
Is Rybelsus the same as Ozempic?
They share the same active ingredient but are not the same product. The doses, the way they are absorbed and the dosing schedules differ, so one cannot be swapped for the other without a doctor’s advice.
Is Rybelsus as effective as Ozempic?
In separate clinical trials, Rybelsus 14 mg daily lowered HbA1c by about 1.4% and Ozempic 1 mg weekly by about 1.6%. The results are close, but tablets only work as expected when taken exactly as directed.
Can I switch from Ozempic to Rybelsus?
Yes, under medical supervision. The doctor will choose a Rybelsus starting dose and monitor your blood sugar after the switch.
Do I need a prescription?
Yes. Both are prescription medicines in Malaysia. DoctorOnCall’s licensed doctors can assess you online and issue a prescription if suitable.
