Rybelsus for Weight Loss: Results, Who It Suits & Better Options

Rybelsus (oral semaglutide) is registered in Malaysia to treat type 2 diabetes. It is not approved as a weight-loss medicine. Many people with type 2 diabetes do lose some weight on it, but the amount is modest, usually a few kilograms.
If your main goal is weight loss, a doctor is more likely to discuss medicines approved for that purpose, such as Wegovy or Mounjaro. This guide covers what Rybelsus can realistically do, who it suits, and how to get the right advice in Malaysia.
What is Rybelsus?
Rybelsus is a once-daily tablet made by Novo Nordisk. Its active ingredient, semaglutide, is the same molecule found in Ozempic and Wegovy injections. It was the first GLP-1 medicine available as a tablet.
Semaglutide copies a natural gut hormone called GLP-1. It works in three ways:
- Helps the pancreas release insulin when blood sugar is high
- Slows how quickly the stomach empties, so you feel full for longer
- Acts on appetite centres in the brain, reducing hunger and cravings
The last two effects explain the weight loss. The tablet comes in 3 mg, 7 mg and 14 mg strengths. The 3 mg dose is a starting dose to help the body adjust; it is not meant for long-term blood sugar control.
How much weight can you lose on Rybelsus?
In clinical trials of adults with type 2 diabetes, people on the highest dose (14 mg) lost around 3–5 kg over six months to a year. That is roughly 3–5% of body weight for most people.
| Study | Who took part | Dose | Duration | Average weight change (approx.) |
|---|---|---|---|---|
| PIONEER 1 | Type 2 diabetes, diet and exercise only | 14 mg daily | 26 weeks | about −3.7 kg (placebo about −1.4 kg) |
| PIONEER 4 | Type 2 diabetes, on metformin | 14 mg daily | 52 weeks | about −4.4 kg |
| OASIS 1 | Obesity, no diabetes | 50 mg daily (not Rybelsus) | 68 weeks | about −15% body weight |
The OASIS 1 result is often quoted online, but it used a much higher dose of oral semaglutide that is not the Rybelsus product sold in Malaysia. Do not expect Rybelsus 14 mg to give similar results.
What affects your result:
- Dose. Most weight loss in trials was seen at 14 mg. Little is expected at the 3 mg starting dose.
- How you take it. The tablet is poorly absorbed if taken with food or too much water.
- Diet and activity. Medicine works alongside lifestyle changes, not in place of them.
- Stopping. Weight often returns after semaglutide is stopped.
Who Rybelsus may suit
Rybelsus is best suited to adults with type 2 diabetes who would benefit from better blood sugar control and some weight loss, and who prefer a tablet to an injection. A doctor may consider it if you:
- Have type 2 diabetes that is not well controlled on metformin or lifestyle changes alone
- Are overweight and want a diabetes medicine that does not cause weight gain
- Are uncomfortable with weekly injections
- Can commit to a strict daily morning routine
Who should avoid it
Rybelsus is not suitable for everyone. Speak to a doctor before starting if you:
- Have a personal or family history of medullary thyroid cancer, or MEN 2 syndrome
- Have had pancreatitis
- Are pregnant, breastfeeding or planning a pregnancy (it should be stopped at least 2 months before trying to conceive)
- Have type 1 diabetes
- Have diabetic eye disease (retinopathy) or severe kidney or stomach problems
- Want to lose weight but do not have type 2 diabetes. Medicines approved for weight management are usually a better fit.
Rybelsus side effects
Most side effects affect the stomach and are strongest in the first weeks or after a dose increase. Common ones include:
- Nausea
- Diarrhoea or constipation
- Vomiting
- Stomach pain or bloating
- Reduced appetite
Less common but serious effects need urgent medical attention: severe stomach pain that spreads to the back (possible pancreatitis), pain in the upper right abdomen or yellowing skin (possible gallbladder problems), signs of an allergic reaction, or changes in vision. Low blood sugar is more likely if Rybelsus is taken with insulin or a sulfonylurea.
How to take Rybelsus
Rybelsus only works well when taken exactly as directed:
- Take one tablet on an empty stomach when you wake up.
- Swallow it whole with no more than half a glass of plain water (up to 120 ml).
- Wait at least 30 minutes before eating, drinking or taking other medicines.
A typical schedule starts at 3 mg daily for 30 days, then rises to 7 mg. Your doctor may increase it to 14 mg after another 30 days if needed. Never adjust the dose without medical advice.
Rybelsus vs Wegovy vs Mounjaro
If weight loss is the main goal, Wegovy and Mounjaro have far stronger evidence. Both are weekly injections studied specifically in people with obesity.
| Rybelsus | Wegovy | Mounjaro | |
|---|---|---|---|
| Active ingredient | Semaglutide | Semaglutide | Tirzepatide |
| How it is taken | Tablet, once daily | Injection, once weekly | Injection, once weekly |
| Developed for | Type 2 diabetes | Weight management | Type 2 diabetes and weight management |
| Average weight loss in key trials (approx.) | 3–5 kg over 6–12 months | about 15% of body weight over 68 weeks | up to about 20% of body weight over 72 weeks |
| Daily routine | Strict: empty stomach, wait 30 minutes | None beyond the weekly dose | None beyond the weekly dose |

The right choice depends on your BMI, whether you have diabetes, your other health conditions and your preference for tablets or injections. A doctor can weigh these with you.

Frequently asked questions
Is Rybelsus approved for weight loss in Malaysia?
No. Rybelsus is registered for type 2 diabetes. Using it for weight loss alone is off-label and should only happen under a doctor’s care.
How fast does Rybelsus work for weight loss?
Most people notice reduced appetite within the first few weeks. Measurable weight loss usually builds over 3–6 months, mainly once the dose reaches 7 mg or 14 mg.
Can I lose weight on Rybelsus 3 mg?
Little weight loss is expected at 3 mg. It is a starting dose designed to reduce stomach side effects before moving to a higher dose.
Is Rybelsus the same as Ozempic?
They contain the same active ingredient, semaglutide. Rybelsus is a daily tablet; Ozempic is a weekly injection. Both are diabetes medicines.
Is Rybelsus better than Wegovy for weight loss?
No. Wegovy was developed and studied for weight management and produced much larger average weight loss in trials.
Will I regain weight after stopping Rybelsus?
Many people regain some weight after stopping semaglutide, especially without ongoing diet and activity changes. Plan the long term with your doctor.
