Take Tolchicine 600mcg Tablet exactly as prescribed by your doctor, usually as soon as possible after a gout flare begins, as earlier treatment tends to work better. Swallow the tablet whole with a glass of water, with or without food, and do not take more than the dose your doctor has prescribed, even if pain is not fully relieved, as exceeding the recommended dose significantly increases the risk of serious side effects.

Dosing for an acute gout attack is typically a higher initial dose followed by a smaller dose some hours later, with the total dose over the following days kept low due to colchicine’s narrow safety margin; for gout prevention, a lower once-daily or alternate-day dose may be prescribed instead. Your doctor will individualise the exact dose and frequency based on your kidney function, age, other medicines and gout history, so do not adjust the dose yourself.

Tolchicine 600mcg Tablet is generally stable for up to 2 to 3 years from the date of manufacture when unopened and stored correctly; always check the expiry date printed on the blister strip and outer packaging and do not use beyond this date.

Store Tolchicine 600mcg Tablet below 30°C in a cool, dry place away from direct sunlight and moisture. Keep in the original packaging until use, and keep out of reach and sight of children, as colchicine overdose in children can be extremely dangerous.

• Severe kidney or liver impairment, unless the dose has been specifically adjusted by a doctor
• Known hypersensitivity to colchicine or any ingredient in the tablet
• Blood disorders such as significant bone marrow suppression
• Concurrent use of certain strong CYP3A4 or P-glycoprotein inhibitor medicines in patients with kidney or liver impairment, due to a serious risk of colchicine toxicity
• Pregnancy or breastfeeding, unless specifically advised as necessary and safe by your doctor

• Certain antibiotics (such as clarithromycin and erythromycin) and antifungal medicines (such as ketoconazole) can significantly raise colchicine levels in the blood and increase toxicity risk
• Some cholesterol-lowering medicines (statins, fibrates) combined with colchicine may increase the risk of muscle-related side effects
• Ciclosporin and certain HIV protease inhibitors can markedly increase colchicine blood levels and generally should not be combined without specific medical guidance
• Grapefruit and grapefruit juice can increase colchicine levels and should generally be avoided during treatment
• Always tell your doctor and pharmacist about every other medicine and supplement you are taking before starting colchicine

• Take the medicine as early as possible after a gout attack begins for the best chance of quick relief
• Never exceed the dose your doctor has prescribed, even if pain persists
• Stop and contact your doctor promptly if severe vomiting, diarrhoea, muscle weakness, numbness, or unusual bruising or bleeding occurs
• Avoid grapefruit juice and check with your pharmacist before starting any new medicine, including over-the-counter products
• Stay well hydrated and follow any dietary advice given for gout, such as limiting alcohol and purine-rich foods
• Keep regular follow-up appointments if colchicine is prescribed long-term for gout prevention

Colchicine has one of the narrowest safety margins of any commonly prescribed oral medicine, meaning the difference between an effective dose and a dangerous one is small, so it should never be taken more frequently or at a higher dose than prescribed, and any leftover tablets from a previous attack should not be used without checking with your doctor first.
