3 Legs Povidone Iodine (Big) is a topical antiseptic solution containing povidone-iodine (PVP-I) at a concentration of 10% w/v, supplied as a clear, amber-brown liquid in a dropper-cap bottle. It is marketed under the long-established ‘3 Legs’ (Cap Kaki Tiga) household antiseptic brand and is sold in Malaysia as an over-the-counter product, without a prescription. The ‘Big’ designation refers to the larger bottle size in this product line (as distinct from the standard smaller dropper bottle sold under the same brand), and the product carries Halal certification.

As a 10% w/v liquid formulation, it delivers a higher concentration of povidone-iodine per application than the lower-strength dry-powder antiseptic sprays sold in the same product category, and it is dispensed as drops or a thin liquid film via the bottle’s dropper cap rather than as an aerosol. It is intended solely for external application to intact or minor superficial skin areas, and is not formulated for internal, ophthalmic, or mucosal use.

Povidone-iodine is a complex of iodine bound to the polymer povidone, which acts as a carrier that gradually releases free iodine on contact with the skin. Free iodine is a broad-spectrum microbicidal agent, acting by oxidising proteins, enzymes and nucleic acids within the cell walls and membranes of a wide range of microorganisms, including bacteria, fungi, viruses and protozoa. At 10% w/v in a liquid vehicle, this formulation delivers a larger total quantity of free iodine per application compared with a lower-strength powder spray, although concentration alone does not determine how quickly the antiseptic effect develops.

Clean and dry the area to be treated before use. Open the dropper cap and apply the solution directly onto the area, or use a clean cotton swab to spread a thin, even layer. Allow the solution to air-dry naturally; do not wipe it off immediately after application. Close the cap tightly after each use to prevent the solution from drying out or spilling. Wash hands before and after application.

This is a topical solution rather than a product administered according to a fixed dose. It is applied directly to the affected external skin area, typically 1 to 3 times a day, or as directed on the product label or by a pharmacist. The amount used should be enough to lightly coat the area without excess pooling of the liquid.

The expiry date is printed on the bottle and outer packaging. No separate unopened shelf-life period beyond this printed date has been independently verified for this product, so the expiry date should always be checked before use.

Store below 30°C in a cool, dry place away from direct sunlight. Keep the bottle tightly capped when not in use to prevent evaporation and discolouration of the solution. Keep out of reach of children. Do not use the product if the solution has changed colour significantly or if the bottle is damaged.

• Common: Mild stinging or a warm sensation immediately after application.
• Common: Temporary brown staining of the skin, and possible staining of surrounding fabric or clothing.
• Serious: Allergic contact dermatitis, presenting as redness, itching, swelling or blistering, which should prompt discontinuation of use and medical advice.
• Serious: Symptoms suggestive of iodine-related thyroid effects (for example, unusual fatigue, palpitations, or neck swelling) with frequent, extensive use, particularly in those with existing thyroid disease.

• Known hypersensitivity to povidone-iodine, iodine, or any other ingredient in the solution.
• People with hyperthyroidism or other active thyroid disorders, unless a doctor has advised otherwise.
• Neonates and infants under 2 years of age.
• Not for use in or around the eyes, in the ears, or on mucous membranes.

• Concurrent application with other antiseptics containing hydrogen peroxide, silver-based, or mercury-based compounds is not recommended, as these may chemically interact with iodine and reduce effectiveness.
• People on long-term lithium therapy, or those undergoing radioiodine-related investigations or treatment, should seek medical advice before regular or extensive use, due to potential effects on iodine-related thyroid monitoring.
• No clinically significant food interactions are known for topical povidone-iodine use.

• Shake the bottle gently before use if the solution appears to have settled.
• Use a fresh cotton swab for each application to avoid contaminating the bottle contents.
• Let the treated area air-dry fully before covering it with a plaster or dressing.
• Keep the dropper cap clean and free of residue to maintain accurate dispensing.
• Check with a pharmacist before applying over large body-surface areas.
• Note the date you first opened the bottle as a personal reminder alongside the printed expiry date.

Precautions: Use with caution on deep wounds, extensive burns, or puncture wounds, where a healthcare professional’s input is recommended first. Limit use in pregnant and breastfeeding women, and use in infants and young children only on medical advice. Avoid prolonged, repeated use over large areas of skin.

When to See a Doctor or Pharmacist: Consult a doctor or pharmacist if the treated area becomes increasingly red, swollen, warm or painful, if pus or fever develops, if a rash appears after use, or if the product is required for longer than 7 days on the same site.
