Overview

Axe Minyak Panas Cap Kapak, also known as Axe Brand Heat Oil, is a topical medicated oil supplied on DoctorOnCall in a 32ml bottle. Its active ingredients are turpentine oil, methyl salicylate, cinnamon oil, cinnamon leaf oil and citronella oil, and it also contains capsicum oleoresin and sanguis draconis (dragon’s blood resin) as inactive ingredients. It is categorised as an Over-the-Counter (OTC) product and is intended for external use on intact skin only.

The name minyak panas means hot oil in Malay, and the formula is a warming type of counter-irritant preparation, unlike the cooling, menthol-and-camphor style of Axe Brand Universal Oil (Minyak Angin Cap Kapak). Methyl salicylate and the essential oils produce a sensation of warmth on the skin, and capsicum oleoresin adds to that warming effect. Axe Brand is a well-known brand that was founded in Singapore in 1928 and is familiar in many households in Malaysia and Singapore.

Because the oil is strongly aromatic and contains methyl salicylate, it is applied in small amounts, a few drops at a time, and gently massaged into the skin. Wash your hands thoroughly after each application and keep the bottle away from the eyes, mouth and other sensitive areas. Check the name and pack size on the carton to make sure you have the heat oil rather than a different Axe Brand product, and keep the bottle out of sight and reach of children.

Directions for Use

Shake the bottle gently if needed, then apply a few drops of the oil to the area of skin and massage it in gently until it is absorbed. Apply 3 to 4 times a day as needed, and do not use it continuously for more than 5 to 7 days.

Apply only to clean, dry and intact skin. Do not apply to broken, irritated, sunburnt or inflamed skin, and avoid the eyes, lips, inside the nose, genital area and other mucous membranes. Wash your hands thoroughly after application.

Do not cover the treated area with a tight bandage, and do not apply a heating pad, hot compress or hot water bottle over it, as this can increase absorption and cause burns. For external use only; do not swallow.

Dosage

Adults: apply a few drops to the area 3 to 4 times daily as needed, with continuous use limited to a maximum of 5 to 7 days.

Children: not recommended without advice from a doctor or pharmacist; do not use on children under 3 years of age.

Maximum: use only a small amount each time and do not exceed 4 applications a day. Missed application: simply apply the next dose when needed; do not apply extra to make up. If the skin becomes irritated, or symptoms continue beyond 5 to 7 days or get worse, stop and consult a doctor or pharmacist.

Storage Instructions

Store in a cool, dry place away from direct sunlight, heat and open flames, with the cap tightly closed. Keep out of sight and reach of children. The oil is flammable because it contains turpentine oil, so keep it away from fire.

Side Effects

  • Common: A sensation of warmth or mild burning at the application site.
  • Common: Mild redness, itching or skin irritation.
  • Common: The strong aroma may cause headache or nausea in sensitive people.
  • Serious: Severe burning, blistering or skin damage where the oil was applied; stop and rinse the area with water.
  • Serious: Allergic contact dermatitis with a spreading rash, swelling or hives; stop use and ask a pharmacist.
  • Serious: Severe allergic reaction with facial swelling or difficulty breathing; seek emergency care.
  • Serious: Signs of too much salicylate absorption with heavy or prolonged use, such as ringing in the ears, nausea, vomiting, rapid breathing or confusion, which need urgent medical attention.

Contraindications

  • Known hypersensitivity to methyl salicylate, aspirin or other salicylates, or to any ingredient such as cinnamon, capsicum, turpentine or citronella.
  • Broken, irritated, sunburnt or inflamed skin, and open wounds.
  • Application to the eyes, lips, inside the nose, genital area or other mucous membranes.
  • Children under 3 years of age.
  • Use with heating pads, hot compresses or tight occlusive bandages over the treated area.

Drug / Food Interactions

  • Warfarin and other blood thinners or antiplatelet medicines: methyl salicylate can be absorbed through the skin and may increase bleeding risk, especially with repeated or large-area use.
  • Oral aspirin, other salicylates and anti-inflammatory tablets: the total salicylate load may add up, so tell a pharmacist if you take them regularly.
  • Other topical products, rubs or patches on the same area: avoid using them together, as this may increase irritation.
  • Heat sources such as heating pads and hot compresses: they can increase absorption and the risk of burns.
  • Food: no food interactions are known.

Quick Tips

  • Use only a few drops at a time and massage in gently.
  • Wash your hands well after every application, and before touching your eyes or face.
  • Do not apply the oil to broken or irritated skin, or cover it with a tight bandage.
  • Test a small area of skin first if you have sensitive skin or have never used the oil.
  • Avoid hot showers, saunas and heat packs immediately after application.
  • Keep the bottle tightly closed, away from flames, and out of the reach of children; the oil may stain fabric.

Relevant Information

Precautions: Ask a doctor or pharmacist before use if you are pregnant or breastfeeding, have an allergy to aspirin or salicylates, take blood-thinning medicines, or have sensitive skin. People with reduced skin sensation, such as those with diabetic neuropathy, should take extra care because burns may go unnoticed. Do not apply to large areas of the body. Children should only use the oil under adult supervision and never under 3 years of age. Keep away from open flames because the oil is flammable. For external use only.

When to See a Doctor or Pharmacist: Stop using the oil and rinse the area with water if you develop severe burning, blistering, a spreading rash or swelling. Seek urgent help for facial swelling, difficulty breathing, ringing in the ears, persistent vomiting or confusion, and contact your local poison information centre or go to hospital if the oil has been swallowed, particularly by a child. See a doctor or pharmacist if symptoms continue for more than 5 to 7 days or get worse, or if you take blood thinners and notice unusual bruising.
