Overview

Counterpain Patch Hot is a topical medicated patch containing menthol, methyl salicylate, and capsicum extract (a chilli-derived extract containing capsaicinoids), applied directly onto the skin as an adhesive patch. It belongs to the category of topical counterirritant/rubefacient patches sold as an OTC product in Malaysian and Singapore pharmacies.

The three active ingredients each contribute a distinct sensory and pharmacological profile: menthol provides a cooling sensation on the skin, methyl salicylate is a salicylate ester related in structure to aspirin, and capsicum extract is derived from chilli peppers and is associated with a warming sensation on the skin — which is why this variant is labelled “Hot” compared with the standard Counterpain Patch.

Counterpain Patch Hot is supplied as individually wrapped adhesive patches, with pack sizes varying by retailer (commonly sold in packs of a set number of patches). As an OTC product, it can be purchased without a doctor’s prescription, though people with sensitive skin, known salicylate or chilli sensitivity, or other health conditions should review the contraindications and precautions below before use.

Directions for Use

  • Clean and dry the skin over the area to be treated before applying the patch.
  • Peel off the protective backing film and apply the adhesive side directly onto the skin, pressing gently to secure it in place.
  • Leave the patch in place for up to 8 hours, then remove; do not reapply a new patch to the same area immediately without a rest period.
  • Wash your hands after handling the patch, and avoid touching your eyes or other sensitive areas until you have done so.
  • Do not apply a heating pad, hot water bottle, or other external heat source over the patch.

Dosage

Counterpain Patch Hot is intended for adults and children over 12 years old. Apply one patch to the affected area up to 2 to 3 times daily, allowing an appropriate rest period between applications as described on the packaging. Do not apply more than one patch at a time unless otherwise directed, and discontinue use if the treated area does not improve within 7 days or if pain worsens.

Shelf Life

Use before the expiry date printed on the individual sachet and outer carton. Once a sachet is opened, use the patch promptly, as exposure to air can reduce the adhesive’s effectiveness and the release of active ingredients.

Storage Instructions

Store below 30°C in a cool, dry place away from direct sunlight. Keep patches in their original sealed sachets until ready to use, and keep out of reach and sight of children and pets.

Side Effects

  • Common: A warming or tingling sensation on the skin shortly after application.
  • Common: Mild, temporary redness at the application site.
  • Uncommon: Itching or a burning sensation, especially on sensitive skin or with prolonged wear.
  • Serious: Blistering, severe burning, or skin peeling, which may indicate a chemical or thermal burn and needs prompt medical attention.
  • Serious: Signs of an allergic reaction such as hives, widespread rash, swelling of the face or lips, or difficulty breathing, which require urgent medical care.

Contraindications

  • Known allergy or hypersensitivity to menthol, methyl salicylate, salicylates (including aspirin), capsicum/chilli extract, or any other ingredient in the patch.
  • Broken, grazed, infected, sunburnt, or recently irritated skin at the intended application site.
  • Children under 12 years old, unless advised by a doctor or pharmacist.
  • Application on the face, eyes, mouth, nose, genitals, or other mucous membranes.

Drug / Food Interactions

  • Avoid applying at the same time as other topical products containing menthol, capsicum, or salicylates over the same area, as this can increase the risk of skin irritation or excess salicylate absorption.
  • Do not use together with a heating pad, hot water bottle, or heat wrap over the same area, as this variant already produces a warming effect and added heat can increase absorption of methyl salicylate through the skin and the risk of burns.
  • People taking oral anticoagulant medicines (such as warfarin) should check with a pharmacist or doctor before regular or large-area use, as topical salicylates can be absorbed into the bloodstream.
  • No specific food interactions are known for this topical product, as it is not taken by mouth.

Quick Tips

  • Perform a small patch test before first full use, especially if you have sensitive skin or have never used a capsicum-containing product before.
  • Wash hands thoroughly after applying or removing the patch.
  • Do not apply immediately before or after a hot shower, bath, sauna, or heating pad use.
  • Remove the patch if you experience excessive burning, stinging, or discomfort rather than trying to tolerate it through the full wear time.
  • Avoid tight, non-breathable clothing over a freshly applied patch.
  • Store unused patches in their sealed sachets, away from direct sunlight and heat.

Ingredients / Active Ingredients

  • Menthol – a topical aromatic compound.
  • Methyl Salicylate – a salicylate ester.
  • Capsicum Extract – a chilli-derived extract containing capsaicinoids.
  • Formulated as a self-adhesive medicated patch for external application.
  • Full inactive ingredient/adhesive-base listing is not publicly disclosed by the manufacturer; refer to the product carton and package insert for the complete ingredient declaration.

Relevant Information

Precautions: People who are pregnant or breastfeeding, elderly individuals, and those with a known salicylate, aspirin, or chilli/capsicum sensitivity should check with a pharmacist or doctor before using Counterpain Patch Hot. Use only on intact skin, avoid large body-surface-area application, and do not use under occlusive dressings or with additional heat sources.

When to See a Doctor or Pharmacist: Stop use and seek medical advice if you develop a rash, blistering, severe burning, swelling, or breathing difficulty after application, if pain persists beyond 7 days, or if you experience unusual symptoms such as ringing in the ears or nausea, which may suggest salicylate absorption.
