Inspra 50mg Tablet

Inspra 50mg Tablet contains eplerenone 50mg, a selective aldosterone receptor antagonist manufactured by Pfizer, used to treat high blood pressure (hypertension) and to improve survival in certain patients with heart failure or heart failure symptoms after a recent heart attack. Because eplerenone raises blood potassium, the risk of dangerously high potassium (hyperkalaemia) is an important safety concern that requires careful monitoring throughout treatment.

Eplerenone works by blocking the action of aldosterone, a hormone that causes the body to retain sodium and water while excreting potassium. By blocking aldosterone’s receptor, eplerenone reduces sodium and water retention, which lowers blood pressure and eases the workload on the heart – but it also causes the body to retain more potassium than usual.

Eplerenone selectively blocks the mineralocorticoid (aldosterone) receptor in the kidney and heart, preventing aldosterone from promoting sodium and water retention and potassium loss. This reduces fluid retention and blood pressure and is thought to protect against harmful tissue remodelling in the heart following a heart attack – but the same mechanism means potassium is retained rather than excreted, which is why blood potassium must be monitored closely.

Take Inspra 50mg Tablet at about the same time(s) each day, with or without food. Swallow the tablet whole with a glass of water. Do not stop taking this medicine suddenly without consulting your doctor, and always attend your scheduled blood potassium tests, as this medicine requires ongoing monitoring for safety.

The dose of Inspra 50mg Tablet is individualised by your doctor based on your condition, blood potassium level, and kidney function, and is usually taken once or twice daily. Your doctor will check your blood potassium before starting treatment, within the first weeks, and periodically thereafter, adjusting your dose if your potassium level rises.

Use this product before the expiry date stated on the packaging. Do not use the medicine if the expiry date has passed.

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

• Known hypersensitivity to eplerenone or any ingredient in this product
• Existing high blood potassium (hyperkalaemia)
• Severe kidney impairment or significantly reduced kidney function
• Severe liver impairment
• Concurrent use with strong CYP3A4 inhibitors, such as ketoconazole or itraconazole, which can significantly raise eplerenone levels
• Concurrent use with potassium supplements or potassium-sparing diuretics unless specifically directed and monitored by your doctor

• Potassium supplements, salt substitutes containing potassium, ACE inhibitors, and angiotensin receptor blockers (ARBs) markedly increase the risk of hyperkalaemia when combined with eplerenone
• Potassium-sparing diuretics (e.g. spironolactone, amiloride) further raise potassium and are generally avoided in combination
• Strong CYP3A4 inhibitors (e.g. ketoconazole, itraconazole, clarithromycin) substantially increase eplerenone blood levels and the risk of side effects
• NSAIDs (e.g. ibuprofen) may reduce the blood-pressure-lowering effect and increase the risk of kidney problems and hyperkalaemia

• Attend every scheduled blood potassium and kidney function test – this is essential for safe use of this medicine
• Avoid potassium supplements and salt substitutes containing potassium unless your doctor has specifically approved them
• Tell your doctor about all other medicines you take, especially other blood pressure medicines and antifungal medicines, due to significant interaction risk
• Report muscle weakness, tingling, or an irregular heartbeat to your doctor immediately
• Rise slowly from sitting or lying down to reduce dizziness
• Do not stop this medicine abruptly without medical advice, particularly if you have heart failure
