Premarin 0.625% Vaginal Cream

Premarin 0.625% Vaginal Cream contains conjugated estrogens formulated for local (intravaginal) use. It is prescribed to relieve vaginal atrophy and dryness caused by the drop in estrogen after menopause, including symptoms such as vaginal itching, burning, discomfort, and pain during intercourse, by restoring moisture and thickness to the vaginal lining at the site where it is applied.

Unlike Premarin oral tablets, which deliver estrogen systemically throughout the body, this cream acts mainly at the local vaginal tissue with comparatively lower systemic absorption, though some estrogen can still enter the bloodstream, particularly with prolonged or higher-dose use. It remains a prescription-only medicine in Malaysia and should only be used after a doctor has assessed your menopausal symptoms and medical history, for example through an online consultation with DoctorOnCall.

Conjugated estrogens applied directly to the vagina are absorbed by the local vaginal tissue, helping to restore the thickness, elasticity, and natural lubrication of the vaginal lining that thins after menopause due to lower estrogen levels, which in turn relieves dryness, itching, irritation, and discomfort during intercourse.

Use Premarin Vaginal Cream exactly as prescribed by your doctor, typically inserted into the vagina using the applicator provided, often at bedtime. Wash your hands before and after use, and wash the applicator with mild soap and water after each use. Your doctor will usually prescribe a higher-frequency regimen for the first few weeks, followed by a reduced maintenance frequency; do not change how often you apply it without your doctor’s advice.

The amount and frequency of application is individualised by your doctor based on your symptoms and response to treatment, and is generally kept to the lowest effective amount used for the shortest duration needed to control symptoms. Do not increase the dose or frequency on your own. If you miss an application, apply it as soon as you remember unless it is close to the next scheduled application.

Premarin Vaginal Cream remains stable until the expiry date printed on the tube and carton when stored correctly and unopened. Once opened, use within the period advised by your pharmacist, and do not use beyond the labelled expiry date.

Store below 30°C (or as stated on the packaging) in a cool, dry place away from direct sunlight. Keep the tube tightly capped when not in use, and keep it out of the reach and sight of children.

• Undiagnosed abnormal vaginal bleeding
• Known, suspected, or past history of breast cancer or other estrogen-dependent cancer
• Current or past history of blood clots in the veins (deep vein thrombosis, pulmonary embolism)
• Current or recent arterial thromboembolic disease such as stroke or heart attack
• Active liver disease or significantly impaired liver function
• Known or suspected pregnancy
• Known hypersensitivity to conjugated estrogens or any ingredient in the cream

• Enzyme-inducing medicines such as rifampicin or certain anticonvulsants, which may reduce estrogen’s effectiveness if the cream is used long-term or at higher doses
• St John’s Wort, which may reduce estrogen effectiveness
• Latex condoms or diaphragms, as the cream’s oil-based components may weaken latex, potentially reducing barrier contraceptive reliability
• Other vaginal products used at the same time, which may affect local absorption or cause additional irritation

• Use the applicator provided and follow your doctor’s instructions on how often to apply the cream
• Wash the applicator after each use and store it clean and dry
• Allow time for the cream to absorb before sexual intercourse to reduce the chance of irritating a partner
