Premarin 0.625mg Tablet

Premarin 0.625mg Tablet contains conjugated estrogens, a mixture of natural estrogen hormones used in hormone replacement therapy (HRT) for women. It is prescribed to relieve moderate to severe menopausal symptoms such as hot flashes, night sweats, and vaginal dryness that result from the natural decline in the body’s own estrogen production after menopause, and may also be used to help prevent postmenopausal osteoporosis in women at significant risk who cannot take other suitable treatments.

As an estrogen hormone tablet, Premarin 0.625mg Tablet is a prescription-only medicine in Malaysia. This is the higher/standard of the two commonly available oral Premarin strengths (the other being 300mcg), commonly used as a standard starting dose for menopausal symptom relief. It should only be started and continued under the supervision of a doctor, for example following an online consultation with DoctorOnCall, because systemic estrogen therapy needs individual assessment of a woman’s medical history, risk factors, and whether the uterus is intact, and requires periodic doctor review.

Conjugated estrogens replace or supplement the estrogen that the ovaries produce less of after menopause. By restoring estrogen levels, the tablet helps reduce the frequency and severity of hot flashes and night sweats, improves vaginal tissue thickness and lubrication, and supports bone density by slowing the bone loss that accelerates after menopause.

Take Premarin exactly as prescribed by your doctor, usually as one tablet daily, swallowed whole with water, at around the same time each day. If your uterus is still intact, your doctor will usually prescribe this together with a progestogen to protect the lining of the womb; do not omit the progestogen component of your regimen unless instructed. Do not stop treatment abruptly without discussing it with your doctor.

The dose is individualised based on your symptoms, medical history, and response to treatment; 0.625mg is this is the higher/standard of the two commonly available oral premarin strengths (the other being 300mcg), commonly used as a standard starting dose for menopausal symptom relief. Your doctor will periodically review whether you still need treatment and will aim for the lowest effective dose for the shortest duration needed to control symptoms. If you miss a dose, take it as soon as you remember on the same day; do not double up the next day’s dose.

Premarin tablets remain stable until the expiry date printed on the packaging when stored correctly and kept in their original blister/container. Do not use the tablets after the expiry date.

Store below 30°C in a dry place away from direct sunlight and moisture, in the original packaging to protect the tablets from light. Keep out of the reach and sight of children.

• Undiagnosed abnormal vaginal bleeding
• Known, suspected, or past history of breast cancer or other estrogen-dependent cancer
• Current or past history of blood clots in the veins (deep vein thrombosis, pulmonary embolism)
• Current or recent arterial thromboembolic disease such as stroke or heart attack
• Active liver disease or significantly impaired liver function
• Known or suspected pregnancy
• Known hypersensitivity to conjugated estrogens or any ingredient in the tablet

• Enzyme-inducing medicines such as rifampicin or certain anticonvulsants (e.g. phenytoin, carbamazepine), which can reduce estrogen’s effectiveness
• St John’s Wort, which may reduce the effectiveness of estrogen therapy
• Thyroid hormone replacement, as estrogen can affect thyroid-binding proteins and may require dose adjustment
• Anticoagulant (blood-thinning) medicines, as estrogen can alter clotting factors
• Grapefruit juice, which may increase blood levels of estrogen
• Corticosteroids, as estrogen may alter how they are metabolised in the body

• Take your tablet at the same time each day to help maintain steady hormone levels
• Attend regular follow-up visits so your doctor can reassess whether you still need HRT
• Keep up with routine breast examinations and mammograms as advised by your doctor
• Avoid smoking while on estrogen therapy, as it increases the risk of blood clots
• Report any unexpected or heavy vaginal bleeding to your doctor promptly
• Seek urgent care for symptoms of a blood clot such as leg swelling, pain, sudden chest pain, or breathlessness

Premarin 0.625mg Tablet is a systemic (oral) form of hormone replacement therapy and works throughout the body, which is different from Premarin 0.625% Vaginal Cream, a local estrogen treatment applied directly to the vagina mainly for vaginal dryness and atrophy with less systemic absorption. Your doctor will help decide which form and strength of estrogen therapy suits your symptoms, and HRT is generally reviewed regularly to use the lowest effective dose for the shortest time needed.
