Overview

Zosaar Hct 50/12.5mg Tablet contains losartan potassium, an angiotensin II receptor blocker (ARB) combined with hydrochlorothiazide, a low-dose diuretic (“water pill”), used to treat high blood pressure (hypertension) and to help protect the kidneys in some patients with type 2 diabetes and kidney disease.

Losartan works by blocking angiotensin II, a hormone that normally narrows blood vessels and raises blood pressure. By blocking its receptor, losartan allows blood vessels to relax and widen while the added hydrochlorothiazide helps the kidneys remove extra salt and water, lowering blood pressure.

How it Works

Zosaar Hct blocks the AT1 receptor that angiotensin II normally binds to on blood vessel walls, preventing the vessel-narrowing and sodium-and-water-retaining effects of this hormone. The added hydrochlorothiazide acts on the kidneys to increase the excretion of salt and water, further lowering blood pressure through a second, complementary mechanism.

Directions for Use

Take the tablet at about the same time each day, with or without food. Do not stop taking it suddenly without your doctor’s advice, even if you feel well.

Dosage

The usual dose is one tablet once daily, but the exact dose is individualised by the treating doctor based on blood pressure response and, when relevant, kidney function and blood electrolyte levels.

Shelf Life

Refer to the expiry date printed on the packaging.

Storage Instructions

Store below 30°C in a dry place, away from direct sunlight and out of reach of children.

Side Effects

  • Dizziness or light-headedness
  • Increased urination
  • Headache
  • Nausea or vomiting
  • Muscle cramps or weakness
  • Sensitivity to sunlight

Contraindications

  • Known hypersensitivity to losartan or hydrochlorothiazide, or to sulfonamide-derived medicines, or any ingredient in the tablet
  • Pregnancy – ARBs can cause harm to the developing baby and should be stopped as soon as pregnancy is confirmed or planned
  • Severe kidney impairment or anuria (inability to pass urine)

Drug / Food Interactions

  • Potassium supplements and potassium-sparing diuretics can raise blood potassium to unsafe levels when combined with losartan.
  • NSAIDs (such as ibuprofen) may reduce the blood-pressure-lowering effect and affect kidney function when combined with losartan or the diuretic component.
  • Lithium levels can rise when combined with losartan or thiazide diuretics, requiring closer monitoring.
  • Inform your doctor of all medicines you are taking, including over-the-counter and herbal products.

Quick Tips

  • Rise slowly from sitting or lying down, especially at the start of treatment, to avoid dizziness from a drop in blood pressure.
  • Attend scheduled blood tests to monitor kidney function and electrolytes as advised by your doctor.
  • Do not double the dose if you miss one; take it as soon as you remember unless it is almost time for the next dose.
  • Tell your doctor immediately if you become pregnant or are planning pregnancy while on this medicine.

Frequently Asked Questions

What is Zosaar Hct 50/12.5mg used for?

It is used to treat high blood pressure, when a single blood pressure medicine alone is not enough to reach target levels.

How does losartan lower blood pressure?

It blocks angiotensin II, a hormone that narrows blood vessels, allowing the vessels to relax and widen, which lowers blood pressure. The added hydrochlorothiazide helps the kidneys remove extra salt and water for an additional effect.

Can I stop taking this suddenly if I feel fine?

No, do not stop suddenly without your doctor’s advice; blood pressure control needs to be continuous even when you have no symptoms.

Can I take this if I am pregnant?

No, this medicine should not be used in pregnancy as it can harm the developing baby; tell your doctor immediately if you are or may become pregnant.

Why do I need blood tests while on this medicine?

Your doctor monitors your kidney function and blood potassium and sodium levels periodically, as this medicine can affect them.

Can I take over-the-counter painkillers with this medicine?

NSAIDs such as ibuprofen can reduce the blood-pressure-lowering effect and affect your kidneys when combined with this medicine – check with your doctor or pharmacist first.

What should I do if I feel dizzy after taking this medicine?

Sit or lie down until the dizziness passes, and rise slowly from sitting or lying positions; tell your doctor if dizziness continues.
