Overview

Axpain 650mg Tablet is a small, film-coated caplet that contains 650mg of paracetamol (also known as acetaminophen) in each caplet. Paracetamol belongs to the analgesic and antipyretic class of medicines. The product is made by Kotra Pharma (M) Sdn Bhd, is categorised as an Over-the-Counter (OTC) product, and is supplied on DoctorOnCall in blister-packed caplets, including an 8-caplet pack.

At 650mg per caplet, Axpain is a higher-strength paracetamol product than the common 500mg tablet, so the number of caplets taken in a day needs closer attention. The maximum daily amount of 6 caplets equals 3,900mg of paracetamol, which leaves little room for other paracetamol-containing products taken on the same day. The film coating makes the caplet easier to swallow and reduces the taste and smell of the medicine.

Paracetamol is found in many combination products, including cold and flu preparations, so always check the ingredient list of any other medicine before taking Axpain. Confirm that the strength on the pack is 650mg and read the leaflet before the first dose. Speak to a pharmacist if you have liver or kidney problems, drink alcohol regularly, have a low body weight, or take other medicines. Keep the pack out of sight and reach of children.

Directions for Use

Swallow the caplet whole with a glass of water. It can be taken with or without food. Do not crush or chew the caplet.

Leave at least 4 hours between doses, use the lowest dose for the shortest time needed, and never exceed the maximum number of caplets in 24 hours.

Do not take Axpain together with any other product containing paracetamol or acetaminophen. If you are unsure whether another medicine contains paracetamol, ask a pharmacist. Avoid alcohol while taking this medicine.

Dosage

Adults and children over 12 years: 1 caplet (650mg) every 4 to 6 hours as needed, or as directed on the pack. Do not exceed 6 caplets (3,900mg) in 24 hours.

Children under 12 years: not recommended, because 650mg is a high-strength caplet. Consult a doctor or pharmacist for a suitable paracetamol product and dose.

Adults with a body weight under 50kg, liver or kidney problems, or regular alcohol intake should take a lower daily amount and speak to a doctor or pharmacist first.

Missed dose: take it when you remember if it is at least 4 hours since the last dose; otherwise skip it. Never double the dose. If symptoms continue for more than 3 days, or get worse, stop and consult a doctor. In case of overdose, seek medical help immediately, even if you feel well.

Storage Instructions

Store in a cool, dry place below 30°C, protected from light and moisture, in the original blister pack. Keep out of sight and reach of children.

Side Effects

  • Common: Nausea or mild stomach upset in some people.
  • Common: Mild skin rash or itching, which is a reason to stop the caplets and ask a pharmacist.
  • Serious: Severe skin reactions with reddening, blistering or peeling of the skin; stop immediately and get emergency help.
  • Serious: Allergic reaction with swelling of the face, lips or throat, or difficulty breathing.
  • Serious: Signs of liver injury such as yellowing of the skin or eyes, dark urine, pale stools, upper right abdominal pain, unusual tiredness or persistent vomiting; this is more likely with overdose.
  • Serious: Unexplained bruising, bleeding or frequent infections, which can point to a rare blood disorder.
  • Serious: Wheezing or tightness in the chest in people who are sensitive to aspirin-type medicines.

Contraindications

  • Known hypersensitivity to paracetamol or any other ingredient in the caplet.
  • Severe liver impairment or active liver disease.
  • Use together with any other paracetamol-containing product.
  • Children under 12 years, unless directed by a doctor or pharmacist.

Drug / Food Interactions

  • Other paracetamol-containing products, including cold and flu medicines and combination pain products: taking them together can lead to accidental overdose.
  • Warfarin: regular daily use of paracetamol can increase the blood-thinning effect, so tell your doctor if you take both.
  • Enzyme-inducing medicines such as carbamazepine, phenytoin, phenobarbital, rifampicin and isoniazid: they can increase the risk of liver injury with paracetamol.
  • Flucloxacillin: used with paracetamol, particularly in people with risk factors, it can raise the risk of a serious blood acid imbalance, so ask a doctor or pharmacist first.
  • Metoclopramide and domperidone can speed up absorption of paracetamol, while colestyramine can reduce it, so leave about one hour between them.
  • Zidovudine: taken with paracetamol, it may increase the risk of a low white blood cell count.
  • Alcohol: regular or heavy alcohol intake increases the risk of liver injury; food does not need to be avoided, as the caplet can be taken with or without food.

Quick Tips

  • Count all the paracetamol you take in a day, including from other products.
  • Keep a note of the time of each dose so you leave at least 4 hours between them.
  • Never exceed 6 caplets in 24 hours.
  • Read the ingredient list of any cold or flu product for paracetamol or acetaminophen.
  • Keep the caplets in the blister pack until needed and store them below 30°C.
  • Check the expiry date, and keep the pack away from children.

Relevant Information

Precautions: Ask a doctor or pharmacist before use if you have liver or kidney disease, drink alcohol regularly, are underweight, malnourished or dehydrated, or have asthma that is sensitive to aspirin-type medicines. Pregnancy: paracetamol at the recommended dose is widely used, but use the lowest dose for the shortest time and speak to a doctor first. Breastfeeding: small amounts pass into breast milk; normal doses are generally considered acceptable, but check with a doctor or pharmacist. Elderly people should take care with the dosing interval. The product is not expected to affect driving. Never exceed the stated dose, as overdose can cause serious liver damage.

When to See a Doctor or Pharmacist: Stop the caplets and seek urgent medical help if you develop a severe skin reaction, yellowing of the skin or eyes, swelling of the face or throat, or difficulty breathing. In case of accidental overdose, go to the nearest hospital emergency department or contact your local poison information centre immediately, even if you feel well. Consult a doctor or pharmacist if your symptoms continue for more than 3 days, get worse, or new symptoms appear.
