Overview

Onchofin 250mg Tablet contains terbinafine 250mg, an allylamine-class oral antifungal medicine used to treat fungal infections of the nails (onychomycosis) and certain skin fungal infections that have not responded adequately to topical treatment.

Terbinafine works by blocking an enzyme called squalene epoxidase that fungi need to build their cell membrane, which is directly toxic to the fungus and leads to its death.

How it Works

By inhibiting squalene epoxidase, terbinafine causes squalene to accumulate inside the fungal cell to toxic levels while starving the cell of ergosterol, the sterol that fungal cell membranes depend on for structural integrity – this dual effect makes terbinafine fungicidal against most dermatophytes. After oral dosing, terbinafine is absorbed and distributed into skin, hair, and nail tissue, where it accumulates and persists at effective concentrations for weeks after the tablets are stopped, which is why visible improvement in a nail infection continues even after the treatment course ends and as the nail grows out.

Directions for Use

Take Onchofin 250mg Tablet at about the same time each day, with or without food, swallowing the tablet whole with a glass of water. Continue for the full duration prescribed by your doctor, even though visible improvement in a nail infection is often slow, since the medicine needs time to clear the infection as the nail grows out.

Dosage

The usual adult dose of Onchofin 250mg Tablet is one tablet (250mg) once daily. Treatment courses are typically several weeks for skin infections and considerably longer – often several months – for nail infections, as your doctor will advise based on which nail is affected and how quickly it is growing; do not stop the course early without checking with your doctor, as this is a course-based treatment rather than an as-needed one.

Shelf Life

Use this product before the expiry date stated on the packaging. Do not use the medicine if the expiry date has passed.

  • Diarrhoea
  • Skin rash or itching
  • Taste changes or loss of taste
  • Abnormal liver function tests
  • Fatigue
  • Muscle or joint pain

Contraindications

  • Known hypersensitivity to terbinafine or any ingredient in this product
  • Active or chronic liver disease
  • Severe kidney impairment (dose adjustment or avoidance may be needed)

Drug / Food Interactions

  • Terbinafine can increase blood levels of medicines metabolised by the liver enzyme CYP2D6, such as certain antidepressants (e.g. tricyclic antidepressants), beta-blockers, and antipsychotics
  • Rifampicin can reduce terbinafine’s blood levels and effectiveness
  • Cimetidine can increase terbinafine’s blood levels
  • Inform your doctor of all medicines you are taking, as well as any history of liver disease, before starting treatment

Quick Tips

  • Your doctor may check your liver function with a blood test before you start treatment and periodically during longer courses, since oral terbinafine can rarely affect the liver
  • Report any yellowing of the skin/eyes, dark urine, or unusual tiredness to your doctor promptly
  • Continue the full prescribed course even if visible improvement seems slow, especially for nail infections
  • Taste changes usually resolve after the course is completed
  • Avoid alcohol or use it only in moderation, as it may add to the burden on the liver during treatment
  • Keep all follow-up appointments so your doctor can assess your response and check for side effects

Frequently Asked Questions

What is Onchofin 250mg Tablet used for?

Onchofin 250mg Tablet is used to treat fungal infections of the nails and certain fungal skin infections that have not responded adequately to topical treatment.

Why does treatment take so long for a nail infection?

A healthy nail must grow out fully to replace the infected part, which can take several months, especially for toenails. Terbinafine continues to act in the nail even after the tablets are stopped, so visible improvement often lags behind the end of treatment.

Do I need blood tests during treatment?

Your doctor may check your liver function before starting and periodically during longer courses, as oral terbinafine can rarely affect the liver. Report any yellowing of the skin/eyes, dark urine, or unusual tiredness promptly.

Can I drink alcohol while taking this medicine?

It is best to avoid alcohol or keep it to a minimum, as both alcohol and terbinafine are processed by the liver, and combining them may add to the burden on the liver during treatment.

What should I do if I miss a dose of Onchofin 250mg Tablet?

Take the missed dose as soon as you remember, unless it is almost time for your next dose – in that case, skip the missed dose and continue your regular schedule. Do not double the dose.

Why has my sense of taste changed since starting this medicine?

Taste disturbance is a recognised side effect of terbinafine and is usually reversible once the course of treatment is completed. Speak to your doctor if it is severe or does not improve after stopping the medicine.
