Overview

Actimol 650mg Tablet is an over-the-counter oral tablet containing paracetamol 650 mg, manufactured by Pharmaniaga Manufacturing Berhad as part of the Actimol range. Paracetamol, also known internationally as acetaminophen, belongs to the analgesic and antipyretic class of medicines and is one of the most widely stocked active ingredients in Malaysian homes and pharmacies. This 650 mg tablet is a single-ingredient product, which means it contains no other active substances and is straightforward to check against other medicines you may be taking.

The 650 mg strength sits above the more commonly seen 500 mg tablet, so the number of tablets allowed per day is lower, and it is important not to apply 500 mg dosing habits to this product. Each tablet is a swallow-whole oral dosage form supplied in blister packs, and the tablets are intended for adults and adolescents aged 12 years and above. Because paracetamol appears in many branded cold, flu and general analgesic products under different names, reading every label for the words paracetamol or acetaminophen is the key safety habit with this medicine.

Pharmaniaga also offers an Actimol 500 mg tablet and an Actimol Menstrual tablet, so please check that the strength printed on the pack matches the product you ordered. As with all OTC medicines, keep the tablets in the original blister strip, out of reach of children, and speak to a pharmacist or doctor before use if you have liver disease, drink alcohol regularly, take other regular medicines, or are pregnant or breastfeeding. DoctorOnCall’s pharmacist team is available if you have questions about whether this strength is suitable for you.

Directions for Use

Swallow the tablet whole with a glass of water. It may be taken with or without food, and it should not be crushed or chewed unless a pharmacist has advised otherwise.

  • Adults and adolescents aged 12 years and above: take 1 tablet (650 mg) every 4 to 6 hours as required.
  • Leave at least 4 hours between doses, and never take more than 6 tablets in any 24-hour period.
  • Do not take this product with any other medicine that contains paracetamol or acetaminophen.
  • Use the lowest dose that is needed for the shortest possible time.
  • Do not give this 650 mg tablet to children under 12 years unless a doctor has specifically advised it.

Dosage

The recommended dose for adults and adolescents aged 12 years and above is 1 tablet (paracetamol 650 mg) every 4 to 6 hours as required.

  • Maximum: 6 tablets in 24 hours, equal to 3,900 mg of paracetamol, which keeps total intake within the 4,000 mg daily ceiling for adults.
  • Missed dose: this is an as-required product, so nothing needs to be made up. Take the next tablet only when it is needed and at least 4 hours have passed since the last one. Never double the dose.
  • Duration: if symptoms continue for more than a few days or get worse, stop taking the tablets and speak to a doctor or pharmacist.
  • Lower daily limits may apply to people with liver disease, regular alcohol use, low body weight or poor nutrition, so seek professional advice first.
  • Overdose: take emergency medical action immediately if more than the recommended amount has been taken, even if you feel well.

Shelf Life

Use before the expiry date printed on the blister strip and the outer carton. Do not use any tablets past this date, and discard tablets that appear discoloured, damaged or crumbled.

Storage Instructions

Store below 30°C in a dry place away from moisture and direct sunlight. Keep the tablets in their original blister strip and carton, and keep all medicines out of the sight and reach of children.

Side Effects

  • Common: Paracetamol is generally well tolerated at the recommended dose, and side effects are uncommon. Mild stomach discomfort or nausea may occasionally occur.
  • Uncommon: Skin rash, itching or hives can occur as a sign of sensitivity to the medicine. Stop taking the product and speak to a doctor or pharmacist if a rash develops.
  • Serious: Swelling of the face, lips, tongue or throat, wheezing or difficulty breathing may indicate a severe allergic reaction, and needs emergency medical care straight away.
  • Serious: Blistering, peeling or widespread red skin rashes, sores in the mouth or around the eyes, or fever with skin symptoms are rare but potentially life-threatening skin reactions that require urgent medical attention.
  • Serious: Taking more than the recommended amount can cause liver damage. Warning signs include yellowing of the skin or eyes, dark urine, pale stools, persistent nausea or vomiting and pain in the upper right abdomen, and may not appear for 1 to 3 days after an overdose, so urgent medical help should be sought even if you feel well.
  • Serious: Unusual bruising, bleeding, frequent infections or persistent sore throat are very rare signs of blood disorders and should be checked by a doctor promptly.

Contraindications

  • Known hypersensitivity (allergy) to paracetamol or to any other ingredient in the tablet.
  • Severe active liver impairment or acute liver disease.
  • Children under 12 years of age, unless specifically advised by a doctor, because the 650 mg strength is not suitable for smaller body weights.

Drug / Food Interactions

  • Duplicate paracetamol: Do not take this product together with any other medicine that contains paracetamol (also labelled acetaminophen), including many multi-symptom cough, cold and flu products, as combined intake can exceed the safe daily limit and harm the liver.
  • Warfarin and other coumarin anticoagulants: Regular, repeated use of paracetamol over several days can enhance the blood-thinning effect and raise the risk of bleeding, so tell your doctor or pharmacist if you are on these medicines.
  • Enzyme-inducing medicines: Medicines such as carbamazepine, phenytoin, phenobarbital, primidone and rifampicin can increase the formation of paracetamol’s liver-toxic by-products, so caution and professional advice are needed.
  • Absorption-altering medicines: Metoclopramide and domperidone may speed up absorption of paracetamol, while colestyramine may reduce it if taken within about an hour of paracetamol.
  • Flucloxacillin: Taking paracetamol together with flucloxacillin, particularly in people with risk factors such as kidney impairment, sepsis or malnutrition, has been linked to a serious blood and body chemistry disturbance called metabolic acidosis, so inform your doctor if you are prescribed this antibiotic.
  • Alcohol: Regular or heavy alcohol intake increases the risk of liver injury with paracetamol; avoid drinking alcohol while using this product.
  • Food: No significant food interaction is known, and the tablets can be taken with or without food.

Quick Tips

  • Always check whether another medicine you take contains paracetamol or acetaminophen before taking an Actimol 650mg Tablet.
  • Remember that this tablet is 650 mg, so the daily maximum is 6 tablets, not the 8 tablets often quoted for 500 mg products.
  • Write down the time of each tablet, or set a phone reminder, so the 4-hour minimum interval is respected.
  • Keep alcohol intake low or avoid it altogether while using paracetamol, as alcohol adds to the strain on the liver.
  • Leave unused tablets in the blister strip, and store the pack in a cool, dry cupboard away from children and pets.
  • If you are ever unsure how many tablets you have taken in a day, wait and speak to a pharmacist rather than guessing.

Frequently Asked Questions

What is the active ingredient and strength in Actimol 650mg Tablet?

Each Actimol 650mg Tablet contains 650 mg of paracetamol, which is also called acetaminophen. It is a single-ingredient tablet from Pharmaniaga Manufacturing Berhad. Please check the label on your pack for the full ingredient list including inactive ingredients.

How many Actimol 650mg tablets can I take in one day?

Adults and adolescents aged 12 years and above may take 1 tablet every 4 to 6 hours as required, up to a maximum of 6 tablets in 24 hours. Keep at least 4 hours between doses, and never exceed the daily limit even if you take other products at different times, because paracetamol from all sources adds up.

Can I take Actimol 650mg Tablet together with other medicines or alcohol?

Do not take it with any other paracetamol-containing medicine. It can interact with certain medicines such as warfarin, some anti-seizure medicines and rifampicin, and regular alcohol use increases the risk of liver injury. Tell our pharmacist team or your doctor about all your medicines and supplements so potential interactions can be checked.

Who should avoid Actimol 650mg Tablet?

It should be avoided by people allergic to paracetamol or any ingredient, people with severe active liver disease, and children under 12 years. People with liver or kidney problems, low body weight, malnutrition, or regular alcohol intake, and those who are pregnant or breastfeeding, should speak to a doctor or pharmacist before taking it.

What should I do if I have taken too many Actimol 650mg tablets?

Seek emergency medical help immediately by going to the nearest hospital emergency department or clinic and taking the pack with you, even if you feel well. Paracetamol overdose can cause serious liver damage that may not show symptoms for one to three days, and early treatment is much more effective.

Relevant Information

Precautions: Check with a doctor or pharmacist before use if you have liver or kidney disease, drink alcohol regularly, are underweight or malnourished, or have been fasting or unwell with vomiting or diarrhoea, as these situations raise the risk of paracetamol toxicity. Paracetamol is generally regarded as an acceptable choice in pregnancy and breastfeeding at the lowest effective dose for the shortest time, but a doctor or pharmacist should still be consulted. Older adults should use the lowest effective dose. Be aware that the 650 mg tablet delivers a larger single dose than 500 mg tablets, so do not substitute one for the other tablet-for-tablet.

When to See a Doctor or Pharmacist: Speak to a doctor if your symptoms last longer than a few days, get worse, or come back soon after stopping. Seek urgent care for rash, facial swelling, breathing difficulty, yellowing of the skin or eyes, dark urine, or unusual bruising or bleeding. If more than the recommended number of tablets has been taken, go to the nearest emergency department immediately and bring the pack.
