Overview

Jardiance Duo 12.5mg/850mg Tablet is a fixed-dose combination tablet containing empagliflozin 12.5mg and metformin 850mg, made up of a sodium-glucose co-transporter-2 (SGLT2) inhibitor and a biguanide. It is used, together with diet and exercise, to improve blood glucose control in adults with type 2 diabetes mellitus.

Empagliflozin works by blocking SGLT2 in the kidney, so more glucose is excreted in the urine instead of being reabsorbed into the blood, while metformin reduces glucose production by the liver and improves the body’s sensitivity to insulin. Combining both mechanisms in one tablet lowers blood glucose through two complementary pathways.

How it Works

Empagliflozin inhibits SGLT2 transporters in the kidney’s proximal tubule, reducing glucose reabsorption and increasing urinary glucose excretion in an insulin-independent manner. Metformin lowers hepatic glucose output (gluconeogenesis), decreases intestinal glucose absorption, and increases peripheral tissue sensitivity to insulin. Together they lower blood glucose through mechanisms that do not rely on stimulating additional insulin secretion.

Directions for Use

Take Jardiance Duo 12.5mg/850mg Tablet with meals to reduce the stomach upset that metformin can cause, swallowing the tablet whole without crushing or chewing it. Maintain adequate fluid intake, particularly during illness, hot weather, or reduced food/fluid intake, since dehydration can affect kidney function and increase the risk of side effects from both components.

Dosage

Jardiance Duo 12.5mg/850mg Tablet is usually taken once or twice daily depending on the specific strength prescribed, with your doctor selecting the combination and frequency based on your current empagliflozin and metformin doses, blood glucose control, and kidney function. Your doctor will individualise and periodically review your dose, including checking kidney function before and during treatment.

Drug / Food Interactions

  • Diuretics may increase the risk of dehydration and low blood pressure when combined with empagliflozin
  • Iodinated contrast media used in imaging studies require metformin to be temporarily withheld due to acute kidney injury and lactic acidosis risk
  • Alcohol increases the risk of lactic acidosis with metformin and should be limited
  • Other glucose-lowering medicines (e.g. sulfonylureas, insulin) may increase the risk of low blood sugar (hypoglycaemia) when combined

Quick Tips

  • Take with food to reduce stomach upset from the metformin component
  • Maintain good hydration, especially in hot weather, during illness, or if you are eating/drinking less than usual
  • Practise good genital hygiene and seek prompt treatment for any signs of a genital or urinary infection

Frequently Asked Questions

What is Jardiance Duo 12.5mg/850mg Tablet used for?

Jardiance Duo 12.5mg/850mg Tablet combines empagliflozin and metformin to help control blood glucose in adults with type 2 diabetes, working through two different mechanisms alongside diet and exercise.

Why does this medicine cause genital infections?

Empagliflozin works by increasing the amount of glucose excreted in urine, which creates an environment where yeast and bacteria can grow more easily around the genital area, increasing the risk of infections such as thrush. Good hygiene and prompt treatment of any symptoms help manage this risk.

Can I take this medicine if I have kidney problems?

Empagliflozin and metformin both depend on adequate kidney function, and this combination is not suitable for people with severe kidney impairment because of a higher risk of lactic acidosis and reduced effectiveness. Your doctor will check your kidney function before starting and periodically during treatment.

Do I need to stop this medicine before a scan with contrast dye or surgery?

Yes, your doctor or the hospital may instruct you to temporarily stop this medicine before and after receiving iodinated contrast dye for an X-ray/CT scan, or before major surgery, because of an increased risk of kidney problems and lactic acidosis. Always tell any doctor treating you that you are on this medicine.

Will this medicine cause low blood sugar (hypoglycaemia)?

Taken alone, this combination has a low risk of causing hypoglycaemia, but the risk increases if it is combined with other diabetes medicines such as sulfonylureas or insulin. Know the signs of low blood sugar (shakiness, sweating, confusion) and how to treat it if it occurs.

What should I do if I miss a dose of Jardiance Duo 12.5mg/850mg Tablet?

Take the missed dose with food as soon as you remember, unless it is almost time for your next dose – in that case, skip the missed dose and continue your regular schedule. Do not take two doses at once.
