My grandma age 90 is having osteoarthritis pain

Hi doctor, my grandma age 90 is having osteoarthritis pain on her knee. She have been taking Tramadol one capsule a day given by someone in the family. It seems to work for her but I have read online regarding withdrawal effects. Im wondering if celebrex would be more suitable? Is tramadol actually fine for her?

Hi, thank you for your question.
Tramadol is a weak opioid used to treat moderate to severe pain. Although it is an opioid, it is less likely to cause opioid side effects, such as breathing problem and constipation. Tramadol is also a drug with low potential for dependence. However, people may become dependent on Tramadol it is taken for more than a few weeks or months.

People with a history of drug abuse are more at risk of developing a dependence on tramadol. Other side effects of Tramadol includes nausea, dizziness, headache, sweating, heartburn, dry mouth and diarrhea.

Celebrex reduces pain and inflammation effectively for osteoarthritis. It causes less gastric ulcer and bleeding compared to nonsteroidal anti-inflammatory drug (NSAID), but these side effects can still occur if Celebrex is taken for a long time. Other side effects of Celebrex are headache, indigestion, diarrhea, nausea and dizziness.
Both these drugs should be used at the smallest possible dose and shortest possible course to avoid adverse effects, especially in older people. You should get advice from your doctor if you wish to start taking any medication.

Osteoarthritis is usually treated with:
o Medications like acetaminophen, nonsteroidal anti-inflammatory drug (NSAIDS) – relieve pain
o Corticosteroid injections – relieve pain
o Hyaluronic acid injections – lubricate joints
o Joint replacement surgery
o Physical and occupational therapy – reduce pain, increase flexibility, strengthen muscles around joint, improve ability to perform activities of daily living

You should always start with medications with the least side effects at its lowest dose possible and only increase the dose or use a stronger drug when the pain worsens.
