I found small wounds on my private part

Hi doctor,I’m 28 Years old Male, and I’m a HIV patient since 1 years back. Im consistently having blood test and medication from Hospital University Malaya, and my result always show good and consistent. Last week I having on and off fever for 5 days, and I found small wounds on my private part… feel a little bit pain on it. My friend told me this is symptoms of syphilis. I have no more fever now, but private part still having small wounds, is there any medication in pill or cream to treat it?

  • Hi there, thank you for the question. Based on your description, you may have some kind of genital sores or lesions. Genital sores or ulcers can be due to an infections or non-infective causes. Common infective causes of genital sores can be genital herpes, syphilis, genital warts and chancroid.

  • Infective causes are generally sexual transmitted diseases. It can be spread through unprotected sexual contact and having multiple sexual partners increases the risk of getting it. Genital herpes has painful ulcers whereas syphilis generally has painless ulcers. The non-infective causes can be due to psoriasis, medications and sexual trauma.

  • Since you had fever that comes along with the small wounds, it may be suggestive of infective causes. Given you are a HIV patient, you may have lower immunity that makes you more prone to infections. However, body immunity is also dependent on current conditions.

  • Attempt to treat the conditions by yourself is not recommended as it may hide the symptoms, making harder for healthcare providers to find out the cause of the problem. During this period of time, it is recommended not to have any sexual activity as it may spread the disease if it is a sexual transmitted diseases.

  • When you have your appointment visits, the healthcare providers will ask you some questions regarding the size and locations of the sores, any previous episodes, or maybe other symptoms such as painful urination or discharge. They may also need to know about your sexual habits.

  • If an infective cause is suspected, the healthcare provider may need blood test to confirm it. Therefore, medications are not recommended and you should seek medical treatment in Hospital University Malaya, as they have your records of appointment.
