Low MCH level in blood test for Thalassemia screening

Hello Dr,

Recently I’m doing blood test for Thalassemia screening. I found out that my MCH level is below range which is 26.

Below is my full HB analysis:

Haemoglobin 163g/L
RBC 6.20 x 10 square 12/L
PCV 0.49 L/L
MCV 78 fL
MCH 26 pg
MCHC 336 g/L
RDW 12.8 %

Ferritin 140 ug/L
Haemoglobin A2 2.7%
Haemoglobin F <0.5%
Hb H Inclusion Bodies Not Detected

What do you think of my test? Do I suspected for thalassemia carrier/traits or iron deficiency ?
I would like to marry with Thalassemia HbE carrier and I do not know if we can proceed if I’m also a carrier.

For your information, Thalassemia is an inherited blood disorder in which the body makes an abnormal form of hemoglobin. Hemoglobin is the protein molecule in red blood cells that carries oxygen. The disorder results in excessive destruction of red blood cells, which leads to anemia. Anemia is a condition in which your body doesn’t have enough normal, healthy red blood cells.

Thalassemia is inherited, meaning that at least one of your parents must be a carrier of the disorder. It’s caused by either a genetic mutation or a deletion of certain key gene fragments. The symptoms of thalassemia can vary. Some of the most common ones include:

  • bone deformities, especially in the face
  • dark urine
  • delayed growth and development
  • excessive tiredness and fatigue
  • yellow or pale skin

According to your lab test result, the MCH value is just slightly below the normal range which is (27-32). It is still on the borderline and you are not said to be a thalassemia carrier just from a lab test without a further history taking and thorough physical examination. Unless your MCH is way below the normal range and you are showing some of the symptoms of thalassemia, then you are advised to refer to a doctor for further treatment.
