Eye shifting when reading or trying to focus

Good day. I noticed that I can’t focus while reading especially on my right eye as I felt that it is out of focus & somehow I felt like my eye is shifting slighty downwards, that I have to closed or blink my eyes to gain focus. I noticed this since last year. What should I do
