Lower abdomen and testis pain

A few years ago, the testicles were sore when pressed through narrow underwear. When masturbating afterwards, there was no ejaculation, just a clear liquid. The testicles and the entire lower abdomen will be painful, the testicles will be red and swollen, feel falling, feel nauseous, and lose appetite. The left lower abdominal pain is more frequent. There is a pipe in the body between the left thigh and lower abdomen, which will cause pain. I have had the habit of holding back urine for several years.
