Gynecomastia, do I need plastic surgery?

Hi Dr, I’m a male aged 27. I have been suffering with gynecomastia since the age of 14. At first I thought it would go away by just exercising, but after 3 years I got no result. I have heard about tamoxifen (Nolvadex) but not sure if I should try it. What do you recommend? Do I need plastic surgery?
