My menstruation is irregular

Hello Dr. im 33 years old and my menstruation is irregular every since im single before. Now i’m a mother of a 11 years old daughter. Then i noticed that my period is to heavy sometimes and its for a long period of time it lasted for 3 weeks including the spotting. It also have a blood clots that before its not that offten. I dont feel dizzy but i always feel sleepy. I never since a Doctor and im planning too.God bless and thank you for reply in advance

Hi there! Thank you for the question. As you have mentioned, you have been experiencing excessive and prolonged bleeding. As to answer your question, there is a possibility you are anemic (deficiency of red blood cells) which causing the sleepiness and it is due to your excessive and prolonged bleeding. There are many causes which can lead to excessive menstrual bleeding.

Most common reason is hormonal imbalance. In a normal menstrual cycle, a balance between the hormones estrogen and progesterone regulates the buildup of the lining of the uterus (endometrium), which is shed during menstruation and if there is hormonal imbalance, the endometrium develops in excess and eventually sheds by way of heavy menstrual bleeding.

A number of conditions can cause hormone imbalances, such as polycystic ovary syndrome (PCOS), obesity, insulin resistance and thyroid problems. Uterine fibroids or also known as noncancerous (benign) tumors of the uterus appear during your childbearing years and usually causes prolonged menstrual bleeding. If your ovaries don't release an egg (ovulate) during a menstrual cycle (anovulation), your body doesn't produce the hormone progesterone, as it would during a normal menstrual cycle and this phenomenon is called dysfunction of the ovaries.

It leads to hormone imbalance and may result in excessive bleeding during menstruation. Polyps, a small, benign growths on the lining of the uterus (uterine polyps) also can cause heavy menstrual bleeding. Adenomyosis is a condition which occurs when glands from the endometrium become embedded in the uterine muscle, often causing heavy bleeding and painful periods.

Menorrhagia is a well-known side effect of using a non hormonal intrauterine device for birth control such as intrauterine device (IUD) and if this is so then you can plan for alternative management options. Cancers such as uterine and cervical cancer also causes excessive menstrual bleeding.

There are some bleeding disorders such as von Willebrand's disease(a condition in which an important blood-clotting factor is deficient or impaired) which can cause abnormal menstrual bleeding. Hence it is advisable to consult a Obstetrician for your condition. This will prevent any future consequences.
