Doctor, my 60 year old aunty is a diabetic since 5 years ago

Doctor, my 60 year old aunty is a diabetic since 5 years ago. She used to be under her family’s direct care and supervision. However, due to certain circumstances, my family have to take over the care for her health condition. I just want to know what do they usually do during a follow up care for a diabetic in hospital? Is there anything I should prepare for her on the day of the follow up?

Hi, thank you for your question.
During a diabetic follow up visit, the nurse or doctor will check your aunt’s blood sugar level, blood pressure and weight. Once a year, they will do a blood test to check for her haemoglobin A1c level and cholesterol levels, and a urine test to look for protein in urine. The doctor will ask her a series of questions about her diet, medications, exercise and whether she is having any new symptoms or illnesses. This is because the doctor will need to know how well her sugar control is and if she has any complications of diabetes.

Therefore, during each visit, you should bring along her record of home blood sugar monitoring and inform the doctor on symptoms of high or low blood sugar. Besides, let the doctor know if she is experiencing any symptoms of kidneys, eyes, nerve, or heart problem, for example, swelling in face or limbs, changes in urine or urination pattern, weight gain, muscle cramps, blurred vision or any visual changes, tingling or burning sensation in the feet, shortness of breath, chest pain, and numbness or weakness on one side of her body. If you have any questions regarding diabetes, you can write them down and ask the doctor on the day of follow up.

The doctor will also examine her eyes, heart, kidneys, skin and feet, to look out for any possible complications. When physical examination is complete, the doctor may decide if your aunt needs adjustments to her medication, give advice on how to keep blood sugar level under control, refer your aunt to another healthcare personnel, such as dietitian, podiatrist, nephrologist or ophthalmologist, and set a date for the next appointment.
