38/M Prolong leg wound, history of HSP/IgA Vasculitis

Non-medicated borderline high cholesterol, non-diabetic, suspected mild thallasemic thru typical blood work, history of high uric acid and potassium, not on long-term medications and no know allergies.

5x5cm wounds on left leg, recovering slowly. 15x20cm wound site on right tibia anterior consist of several island wounds, not improving. Symptoms started many months back, signs of improvements on several occasions but never fully-healed. Daily dressing change every 24hrs, rinse with antibacterial sulphur soap during shower, pad dry with gauze, irritate with normal saline with debridement of slough, followed by topical applicantion of dermacyn and providone before covering with Melolin and crepe bandage. When required, oral paracetamol and voltaren 50mg for pain management. Prescribed propase for a week with notable improvements, but condition returned after that. Right leg wound discharge soaking thru dressing in deep red color with strong odor. No fever, no impact to motor or sensory functions, dark discoloration of skin around wound site.

Fear of hospital, hence hoping to get effective (possibly aggressive at this stage) medical assistance to permanently resolve this prolonged condition, both oral medication and wound management. Any form medical alternatives, cost is immaterial, so long as does not involve hospital visits would be much-appreciated. So thank you in advance for the attention and assist.
