Hi there! Thank you for your question. As you have mentioned, you have missed your period for about 4 weeks and you are not pregnant as well. As to answer your question, there are many reasons which can cause menstrual irregularities but it usually due to hormonal or structural abnormalities.

First of all let me begin by defining menstrual irregularities. A menstrual cycle which ranges from 21 to 35 days is considered a normal cycle. So if you are experiencing a longer or a shorter cycle then you are having an abnormal menstrual cycle which needs medical attention.

An important cause of menstrual irregularities is hormonal imbalance. Estrogen and progesterone are the two hormones that regulates your cycle and if they get out of balance it will cause delayed menstruation or spotting. Polycystic ovary syndrome (PCOS) is a hormonal disorder and it is a common condition in this generation. As you have mentioned that you have been experiencing menstrual irregularities many times in the past, it could be a symptom of PCOS.

In PCOS, the ovaries may develop into numerous small collections of fluid (follicles) and fail to regularly release eggs causing menstrual irregularities.
There are few tests you can do to confirm the reason for your menstrual irregularities such as pelvic examination, pap smear (rule out vaginal cancer), blood test (can determine if there is any hormonal imbalance) and ultrasound (to identify if there are any structural abnormalities within your uterus).

Once the reason for your menstrual irregularities is established then it would be easy to treat the root cause.
Hence it is advisable to consult a obstetrician as soon as possible for your condition. This will prevent any future consequences.
