Identification Problems

Im a teenager who has been forgotten my identity lately. It actually started when I were younger but it came back. I felt like Im in someone’s body. I felt like Im a soul. I forget my name and even how I look like. Not to mention I forget things I want to do or something in a sec. People said it causes of stress. Is it true? Should I be concerned?
