Red penis and scrotum in toddler

18 month old boy. Penis and scrotum looks red. No redness anywhere else. Scrotum constantly tight. Sensitive to touch and water. No pain when peeing. Only painful when in contact with water or diapers/pants, ie no pain when he is running around if not wearing anything. Just pooped (watery). Suspect that poop was acidic or that the disposable diapers was abrasive. Any need to worry? What can i do/apply to ease the pain? Especially since i need to let him wear something to sleep. How long before he heals?
